摘  要:目的观察艾贝宁对急性左心衰竭交感活性亢进的抑制作用。方法 137例急性左心衰竭患者随机分为对照组(C组)、倍他乐克组(M组)、艾贝宁组(D组),测定3组治疗前及治疗3 d和5 d后血浆去甲肾上腺素(NE)、肾上腺素(E),治疗前及治疗3 d血浆脑型利钠肽(BNP)水平,并行脉搏指示连续心排血量监测心排血量(CO)、心指数(CI)、血管外肺水(EVLWI)等指标评估心功能状态,监测氧和并了解艾贝宁对呼吸的影响。结果与C组比较,D组血浆NE、E及BNP水平明显降低(P均<0.05),血流动力学CO、CI、EVLWI、氧分压明显升高,肺泡-动脉氧分压差明显下降,且不影响呼吸。结论艾贝宁可有效抑制心力衰竭交感活性亢进,改善心功能而不影响呼吸。Objective It is to observe the effects of anti-sympathetic activity of dexmedetomidine in acute left ventricular failure. Methods 137 cases of acute heart failure were randomly divided into control group (C) , metoprolol group ( M ) , dexmedetomidine group (D). The levels of plasma noradrenaline (NE) , epinephrine (E) before treatment and in 3 d, 5 d af- ter treatment, the levels of plas parallel pulse indicator continuous cardiac output monitoring (PiCCO) monitoring of cardiac output (CO), cardiac index (CI), extravascular lung water (EVLWI) were detected to evaluate cardiac function status, blood oxygen was detected and the effect of dexmedetomiding on breath was observed. Results Compared with group C, the levels of NE, E and BNP were significantly lower, the levels of CO, CI, EVLWI and oxygen partial pressure and were higher in group D (P 〈 0.05) with lower alveolar-arterial oxygen, and the respiratory function was not inhibited. Conclusion Dexme- detomidine can effectively inhibited sympathetic activity and improved heart function without any side effect on respiratory function.
