I类切口手术预防用抗菌药物的干预研究

摘　　要：目的比较干预前、后某院I类切口手术预防用抗菌药物的合理性，考察干预措施的实施效果。方法将某院2010年12月出院I类切口手术患者1140例为非干预组，2012年12月出院I类切口手术患者1148例为干预组，比较2组I类切口手术预防用抗菌药物的合理性。结果干预后，抗菌药物使用率下降了41．6％（P〈0．05），第1代头孢菌素应用率提高了30．5％（P〈0．05）；初次用药时机在术前0．5—2h的比例提高了33％（P〈0．05），术后用药时间≤48h的比例提高了25．4％（P〈0．05）；干预前后2组均无一例术后感染。结论实施的干预措施具有可行性和有效性，对促进安全、有效、合理地应用抗菌药物起到了积极作用，但还需加强对抗菌药物合理应用的培训及规范化管理。Objective To compare the rationality of prophylactic application of antimicrobial drugs in type I incision operation in our hospital before and after intervention to investigate the effects of intervention measures. Methods 1140 discharged patients in our hospital during Dec. in 2010 were included in non -intervention group and 1148 discharged patients in our hospital during Dec. 2012 were included in intervention group. The rationality of prophylactic application of antimicrobial drugs in type I incision operation was compared between two groups. Results After intervention ,the application of antimicrobial drugs decreased by 41.6% （P 〈 0.05 ）, and the application of first - generation eephalosporins antibiotics increased by 30.5 % （ P 〈 0.05 ）. The percentage of initial medication 0.5 - 2 h before incision increased by 33% （P 〈 0.05 ）. The percentage of medication time after operation which was less than 48 h increased by 25.4% （ P 〈 0.05 ）. No patients infected after operation in two groups. Conclusion The pharmaceutical intervention of our hospital is feasible and valid to promote safe, effective and rational use of antimicrobial drugs, but it is still need to continue to strengthen the reasonable application of training on antimicrobial drugs and standardization management.
