摘  要:目的 探讨重离子等5种不同辐射技术在瘢痕疙瘩美容治疗中的方法及临床疗效.方法 对208例235处瘢痕疙瘩采用5种辐射技术治疗,并进行疗效分析.结果 重离子束照射12例14处,均达治愈标准;手术切除+电子线照射治疗101例119处,1年后复发11例7处;单纯电子线照射69例73处,治愈71处,好转1例,1年后复发8例9处;^192 Ir贴敷12例14处,10例11处治愈,好转1例,无效1例3处,1年后复发4例;32P注射14例15处,12例12处治愈或好转,2例3处无效,1年后复发4例.结论 重离子辐射技术在瘢痕疙瘩美容性治疗中有不可或缺的地位,疗效好,无明显不良反应,但因价格昂贵,一般患者难以承受;手术+电子线照射治疗为最佳选择,既经济实用,又无色素沉着等并发症,美容效果好,还可有效防止术后复发,并且创口愈合无明显瘢痕.Objective To evaluate the clinical effect of five different radiotherapies on keloid.Methods A total of 208 patients with 235 lesions received different radiotherapies.Results Fourteen lesions in 12 patients received heavy ion radiotherapy.All of them reached complete recovery.119 lesions in 101 patients received surgery plus conventional radiotherapy.After 1 year,11 patients relapsed.73 lesions in 69 patients received electron beam irradiation,and 71 lesions recovered.1 patient was improved and 9 lesions in 8 patients relapsed after 1 year.12 patients received brachytherapy by 192 Ir on 14 lesions.11 lesions in 10 patients recovered,but 1 patients lost efficacy on 3 lesions.After 1 year,4 patients relapsed.14 patients received nuclide 32p application on 15 lesions.12 lesions in 12patients recovered while 2 lesions in 3 patients were of no effect.After 1 year,there were 4 patients relapsed.Conclusions Heavy ion beam has the characteristics of better efficacy and fewer side effects,but the therapy cost is very high.Surgery and electron beam irradiation is the first choice for keloid with economical cost and good efficacy.
