摘  要:目的探讨良性阵发性位置性眩晕(BPPV)患者前庭双温交替试验的特点。方法采用前庭双温交替试验法检测57例BPPV患者(BPPV组)及60名正常对照者(正常对照组)的最大慢相速度和半规管轻瘫(CP)发生率,并对结果进行比较分析。结果 BPPV组眼震最大慢相速度[(14.13±14.22)°/s]与正常对照组[(13.69±12.35)°/s]比较,差异无统计学意义。BPPV组CP 28例(49.1%),正常对照组CP 8例(13.3%),二者比较差异有统计学意义(P<0.01)。BPPV组患侧CP发生率明显高于健侧(P<0.05)。后半规管BPPV、水平半规管BPPV及上半规管BPPV患者CP发生率比较,差异无统计学意义。结论 BPPV组患者CP发生率增高,且患侧的CP发生率明显高于健侧。前庭双温交替试验有助于BPPV患者的定位诊断。Objective To investigate the characteristics of alternate bithermal caloric test in patients with benign paroxysmal positional vertigo (BPPV). Methods The slowphase velocity and incidence of canal paresis (CP) of 57 BPPV patients (BPPV group) and 60 healthy controls (normal control group) were detected by alternate bithermal caloric test. And the results were analyzed. Results There was no significant difference of slowphase velocity between BPPV group [ (14.13 ± 14.22)°/s] and normal control group [ (13.69 ±12.35)°/s]. There was 28 CP (49.1%) in BPPV group, and 8 CP ( 13.3% ) in normal control group, and the difference between the two groups was statistical significant (P 〈 0. 01 ). The incidence of CP at affected side in BPPV group was significantly higher than that at healthy side ( P 〈 0.05 ). There were no significant differences of incidence of CP among superior semicircular canal, horizontal semicircular canal and posterior semicircular canal in BPPV group. Conclusion The incidence of CP in BPPV patients is increased, and which at affected side was significantly higher than healthy side. The alternate bithermal caloric test can be helpful to location diagnose BPPV.
