罗库溴铵的不同稀释浓度对女性静脉注射痛的影响

摘　　要：目的：采用生理盐水稀释预注剂量的罗库溴铵并观察其对清醒状态下女性患者静脉注射痛的影响。方法：选取择期行腹部手术的女性患者90例，ASA分级Ⅰ-Ⅱ级，年龄20-50岁，体质量40-80kg。将所有患者随机分为Ⅰ组，Ⅱ组，Ⅲ组，每组30例。3组均在清醒状态下于5s内预注罗库溴铵0．06mg·kg^-1。Ⅰ组药液未经稀释，质量浓度为10mg·ml^-1；Ⅱ组和Ⅲ组用生理盐水分别稀释至1mg·ml^-1和0．5mg·ml^-1。测定3种预注药液的渗透压和pH值，记录疼痛评分、起效时间、插管条件、抑制时间。结果：Ⅰ组总的疼痛发生率为80％，而Ⅱ组和Ⅲ组分别为40％和3．3％，与Ⅰ组比较差异均有统计学意义（P〈0．05），Ⅱ组、Ⅲ组之间比较差异亦有统计学意义（P〈0．05）；Ⅱ组、Ⅲ组中均未见重度疼痛发生，与Ⅰ组（30％）比较差异均有统计学意义（P〈0．05）；3组患者起效时间、插管条件、抑制时间比较差异均无统计学意义（P〉0．05）。结论：将罗库溴铵稀释成质量浓度为1mg·ml^-1或0．5mg·ml^-1。后均可降低女性患者罗库溴铵静脉注射痛的发生，并且浓度越低效果越明显，是一种既简单又经济的方法。OBJECTIVE To observe the effect of dilution of rocuronium with normal saline on the frequency and severity ofpain during intravenous injection in awake female patients. METHODS Ninety ASA Ⅰ or Ⅱ women undergoing surgical pro cedures that required general anesthesia were randomized into three groups. Group Ⅰ received rocuronium 1（I mg·mL^-1 ,Group Ⅱ and Ⅲ received 1 and 0. 5 mg·mL^-1 of rocuronium, respectively. Osmolality and pH of the solutions, pain scores, onset time, intubating conditions, duration of action were recorded. RESULTS The overall frequencies of pain were 80% in Group Ⅰ , 40% in Group Ⅱ , and only 3.3% in Group Ⅲ. In group Ⅱ and Ⅲ, the incidence of pain were significantly less when com pared with that of Group I（P〈0. 05）,and there was also significant difference between group Ⅱ and Ⅲ（P〈0. 05）. 30% pa- tients in Group Ⅰ had severe pain,but there were no complaints for that in both Group Ⅱ and Ⅲ （P〈0. 05）. With regard to the onset time, intubating conditions and duration of action, there were no significant differences among three groups（P〉0. 05）. CONCLUSION In awake female patients, dilution of rocuronium 10 mg·mL^-1 at small doses to Ⅰ or 0. 5 mg·mL^-1 with nor- mal saline is a simple and cost-effective strategy for preventing pain during intravenous injection.
