摘  要:目的探讨丙泊酚-异氟醚复合全麻对急性脑出血(ACH)患者脑氧代谢的影响。方法在丙泊酚静-吸复合全麻下行急诊开颅血肿清除术ACH患者78例,记录术前术中的MAP、HR、血氧饱和度(SO2),计算动-静脉血氧含量差和脑氧摄取率(CEO2)。结果患者术中MAP、HR均较术前改善(P<0.05)。与术前比较,术中CEO2增加[(0.24±0.08)vs.(0.36±0.15)](P<0.05),脑氧合状况改善(P<0.05)。结论丙泊酚具有扩张周围血管、降低血管阻力、降低CEO2和改善脑氧合等优点,可作为ACH患者急诊开颅血肿清除术的麻醉选择。Objective To observe the effect of combined propofol and isoflurane anaesthesia on cerebral oxygen metabolism in the patients undergioing evacuation of acute cerebral hematoma (ACH). Methods Seventy-eight ACH patients received cerebral hematoma evacuation under combined propofol and isoflurane anaesthesia. The mean arterial pressure(MAP), heart rate(HR) and blood oxygen saturation were recorded and the defference of arteriovenous oxygen content and cerebral oxygen extraction rate (CEOz) were calculated before and during operation. Results Compared to before,the MAP and HR during operation were improved remarkably (P〈0. 05) and CEOz was increased significantly[(0. 24±0. 08) vs. (0.36±0. 15)] (P〈0. 05). Conclusion Combined propofol and isoflurane anaesthesia has the advantages of reducing peripheral vascular resistance and improving cerebral oxygen metabolic status in ACH patients undergioing evacuation of cerebral hematoma.
