摘　　要：目的介绍“围巾式”食管－胃吻合术预防术后吻合口瘘的临床经验。方法回顾性分析2002年1月至2014年1月对75例门静脉高压症并食管下段静脉曲张出血病例行食管下段及胃底切除术时使用“围巾式”食管－胃吻合术治疗的结果。观察术后并发症发生率和病死率。结果75例手术无吻合口瘘发生，吻合口狭窄3例，术后复发出血2例，手术死亡1例。结论“围巾式”食管－胃吻合术可减少食管下段及胃底切除术后吻合口并发症，安全、有效。Objective To summarize institutional experiences of scarf-like gastroesophageal anastomosis for preventing anastomotic leak. Methods Analyses were performed for the clinical outcomes of 75 consecutive patients undergoing scarf-liking gastroesophageal anastomosis after lower esophagus-proximal gastric resection in portal hypertension with lower esophagus variceal bleeding from January 2002 to January 2004. Results No anastomotic leak was found. Anastomotic stricture oc- curred in 3 patients, another 2 had postoperative re-bleeding and one died. Conclusions The novel scarf-like gastroesophageal anastomosis is both safe and effective and it may reduce anastomotic com- plications after lower esophagus-proximal gastric resection.
