41例腹腔镜下肾癌根治术肾旁前间隙入路解剖学的应用

摘　　要：目的研究肾旁前间隙入路解剖学及腹膜后筋膜间隙分层在腹腔镜下肾癌根治术中的应用,探讨其在后腹腔镜下筋膜外腹腔镜下肾癌根治术中的作用。方法选择肾旁前间隙入路作为肾前方的手术平面的手术。对41例患者行后腹腔镜下肾旁前间隙入路肾癌根治术。结果 41例患者手术均成功,手术平均时间(90±35)min,手术出血量(50±30)mL,术中未出现周围脏器及大血管损伤等并发症,平均住院(5±3)d。结论腹腔镜下肾旁前间隙入路肾癌根治术具有容易组织游离、解剖层次清楚、手术中出血量少、手术时间短、术后恢复快、并发症少等优点,肾旁前间隙入路可作为肾癌根治的手术平面。Objective To study the beside the renal anatomy before clearance and retroperitoneal fascia layer in laparoscopic kidney cancer radical prostatectomy, to discuss curative effect of laparoseopie kidney cancer radical prostatectomy under laparoscopie fascia. Methods Choosing the operation of radical nephrectomy beside the renal anatomy before clearance under laparoscope. 41 eases had lapa- roscopic radical nephrectomy beside the renal anatomy before clearance under laparoscope. Results All the operations were successful. The mean operation time was （90 ±35 ） rain, the mean intraoperative blood loss was （ 50 ± 30 ） mL with no complications. The average hospital stay was（ 7 ± 3 ）days. Conclusion Laparoseopic radical nephrectomy beside the renal anatomy before clearance under laparoscope is easy to organize freely, and has the characteristics of clear anatomical level, less bleeding in operation, shorter operation time, faster postopera- tive recovery and less postoperative complications. Laparoscopic radical nephrectomy beside the renal anatomy before clearance under lapa- roscope can be used as the surgery level of radical nephrectomy.
