摘 要:目的应用实时三平面组织同步显像技术评价前壁和下壁心肌梗死患者收缩不同步运动。资料与方法 50例正常人、25例前壁心肌梗死患者及25例下壁心肌梗死患者应用实时三平面组织同步显像技术测量12节段收缩达峰时间(Ts)及收缩达峰速度(Vp),并得到12节段收缩达峰时间标准差(Ts-SD)和12节段的最大、最小时间差值(Ts-diff)。结果①与对照组相比,前壁心肌梗死组和下壁心肌梗死组Ts明显延长(P<0.05),前壁心肌梗死组前壁和前间隔Ts明显长于其他节段(P<0.05),下壁心肌梗死组下壁和后壁Ts明显长于其他节段(P<0.05)。②前壁心肌梗死组和下壁心肌梗死组Vp较对照组明显减低(P<0.05)。③前壁心肌梗死组和下壁心肌梗死组不同步指数Ts-diff、Ts-SD较对照组显著延长(P<0.05)。④左心室射血分数与Ts-diff、Ts-SD呈显著负相关(r=-0.652、-0.552,P<0.05)。结论心肌梗死患者存在左心室收缩功能损害及不同步运动,且两者呈显著负相关,三平面组织同步显像技术为准确地评价不同部位心肌梗死患者的左心室功能及收缩不同步运动提供了新方法。PurposeTo evaluate the left ventricular systolic dyssynchrony in patientswith anterior and inferior myocardial infarction by real-time triplane echocardiographytissue synchronization imaging. Materials and Methods Fifty health adults and 50 MIpatients (25 anterior MI patients and 25 inferior MI patients) were enrolled in the study.The time to regional peak systolic velocity (Ts) and the peak systolic velocity (Vp) in12 segments of the left ventricular were measured semi-automatically by using realtimetriplane echocardiography tissue synchronization imaging and two parameters ofsystolic dyssynchrony (Ts-diff and Ts-SD) were calculated. Results ① In MI patients, Tswas significantly prolonged (P〈0.05). In anterior MI patients, Ts was longer in anteriorsegment and anterior septal than other segments and in inferior MI patients, it waslonger in inferior segment and posterior segment (P〈0.05). ②Vp significantly decreasedin MI group, compared with that in control group (P〈0.05). ③Both Ts-SD and Ts-diffwere prolonged in the anterior MI group and inferior MI group compared with thosein control group (P〈0.05). ④ Left ventricular ejection fraction correlated negatively withdyssynchrony indexes Ts-diff and Ts-SD (r= -0.652, -0.552; P〈0.05). Conclusion MIpatients undergo both systolic dyssynchrony of LV and systolic function damage, and leftventricular ejection fraction correlates negatively with the of dyssynchrony parameters.Real-time triplane echocardiography tissue synchronization imaging is a novel methodwhich can accurately assess left ventricular dyssychrony in various sites as well as leftventricular function in MI patients.
