摘  要:目的 探讨高频超声结合增强型能量多普勒(E-flow)显像技术评价糖尿病患者指端微血管血流的临床价值.资料与方法 90例糖尿病患者按糖尿病病程分为糖尿病≤ 5年组(30例)、糖尿病6~10年组(30例)和糖尿病>10年组(30例),选择30例健康体检者作为对照组;均行高频超声检查,并结合E-flow显像观察受试者右手中指末节指端微血管血流分布特点,测量其血管/甲床像素比.结果 对照组右手中指末节指端血流信号丰富,指掌侧固有动脉、指腹动脉、甲床动脉及其细小分支显示清晰;糖尿病各组患者中指末端血流显示欠清晰,连续性不佳,血流分布明显减少.对照组、糖尿病≤5年组、糖尿病6~10年组、糖尿病>10年组血管/甲床像素比分别为0.1640±0.0335、0.1040±0.0140、0.0791±0.0079、0.0555±0.0182.与对照组比较,糖尿病各组血管/甲床像素比均减低(F=100.99,P<0.05),且随糖尿病病程的延长减低更明显.结论 高频超声结合E-flow显像技术可以敏感、准确地反映患者指端微血管血流的变化,为临床评价糖尿病患者微血管病变提供依据.PurposeTo explore the clinical value of high frequency ultrasonographycombined with E-flow in evaluating fingertip microvascular blood flow changes in patientswith diabetes. Materials and Methods Ninety patients with diabetes were classified intothree groups according to the course of disease group A ( ≤ 5 years diabetes, n=30),group B (6-10 years diabetes, n=30) and group C (more than 10 years diabetes, n=30).Thirty volunteers were selected as the controls. All the subjects had high frequencyultrasonography combined with E-flow imaging to display the distribution of smallvessels in the right middle fingertips. The pixel ratio of the vessels and the nail bed wascalculated. Results In the control group, blood flow was rich, and the palmar arteries,pulp arteries, the hyponychium arteries and vascular net of fingertip displayed clearly andcontinuously by E-flow imaging. In diabetes groups, the fingertip's small vascular flowshowed less clear, poor continuity, and decreased blood flow distribution when comparedwith the control group. The pixel ratio of the vessels and the nail bed in group A, B and C (0.1640±0.0335, 0.1040±0.0140, 0.0791±0.0079) was lower than that in the controlgroup (0.0555±0.0182) (P〈0.05), which was more obviously with the prolongation of thecourse of diabetes. Conclusion High frequency ultrasonography combined with E-flowimaging is sensitive and reliable to reflect fingertip's microcirculation changes, whichprovides a new method to assess microvacular flow changes in patients with diabetes.
