米力农治疗慢性肺源性心脏病合并心力衰竭临床疗效分析

摘　　要：目的：观察米力农治疗慢性肺源性心脏病合并心力衰竭的临床疗效。方法将48例慢性肺源性心脏病合并心力衰竭患者分为2组：对照组予以氧疗、抗感染、化痰、平喘、利尿、维持电解质平衡等常规治疗。治疗组在常规治疗基础上加用米力农10 mg于生理盐水250 ml，静脉滴注，于2～3 h内输完，1次/d，5 d为1疗程，观察临床疗效，治疗前后左室射血分数（ LVEF）、每分心排血量（ CO）、每搏量（ SV）、肺动脉压（ PAP）的变化及不良反应。结果治疗组总有效率为91．6％，对照组总有效率为62．5％。2组疗效比较差异具有统计学意义。治疗组患者治疗后的心脏指数、左室射血分数、肺动脉压明显改善，与对照组患者相比差异具有统计学意义。结论在常规治疗基础上加用米力农治疗慢性肺源性心脏病合并心力衰竭是安全有效的，疗效肯定。Objective To observe the clinical effect of milrinone in treatment of chronic pulmonary heart disease combined with heart failure .Methods 48 cases of chronic pulmonary heart disease with heart failure patients were divided into two groups.The control group was given oxygen therapy ,anti-infection,eliminating phlegm,relieving asthma,diuresis,maintaining electrolyte balance and other conventional treatment .The treatment group , beyond the conventional treatment , was dealt with milrinone infusion 10mg in physiological saline 250ml, within 2~3H, 1/d, 5D for a course of treatment .All observed were clinical curative effect before and after treatment ,left ventricular ejection fraction （LVEF）,cardiac output（CO）,stroke volume （ SV） ,pulmonary artery pressure （ PAP） changes and adverse reactions .Results The total effective rate of the treatment group was 91.6%in contrast with the control group＆#39;s 62.5%.There was significant difference in the efficacy of the two groups .After treatment ,the cardiac index , left ventricular ejection fraction ,and pulmonary artery pressure of the treatment group significantly improved compared with the control group .The difference was statistically significant .Conclusion On the basis of convention-al therapy , the treatment of chronic pulmonary heart disease combined with heart failure with milrinone is safe and effective .
