摘 要:背景近红外光谱技术(near-infrared spectroscopy,NIRS)是一种能够穿透颅外组织检测脑组织血氧饱和度(regional cerebral oxygensaturation,rSO2)的技术,具有无创、连续、实时的特点。目的综述NIRS在成年人心脏外科手术脑功能监测中的应用研究进展,为其在临床研究及实践中的应用提供参考。内容分别从心脏外科手术预后评估、术中干预指导及术前风险评估等方面综述近年病例报道、观察性研究及实验性研究结果,分析各研究间存在的争议及其原因。趋向NIRS在心脏外科围手术期监测中拥有广泛的应用前景,但尚需更多高质量的证据指导临床实践应用。Background Near-infrared spectroscopy (NIRS) can detect the regional cerebral oxygen saturation (rS02) penetrating the extra-cranial organization, which has such features as of being non-invasive, continuous and real-time. Objective The paper reviews the research progress of the application of NIRS technology to brain function monitoring in the adult cardiac surgery, and provide practical guidance for clinical anesthetists. Content From the aspects of prognostic evaluation, intraoperative intervention guidance and preoperative risk stratification, we reviewed recent case reports, observation studies and experimental studies, as well as existing dispute and its possible reasons. Trend NIRS has broad application prospects in cardiac surgery perioperative monitoring, while in terms of specific clinical guidance, more high-level evidences are needed.
