摘　　要：背景盐酸右美托咪定（dexmedetomidine，Dex）是一种高选择性的α2肾上腺素能受体α2 adrenoreceptor，α2AR）激动剂，具有镇痛、镇静、抗焦虑、抗交感神经活动等作用。已有多项动物实验证实Dex鞘内应用的安全性。目的回顾Dex在临床鞘内应用的研究进展，为临床应用提供指导。内容阐述Dex鞘内应用的安全性以及对局麻药作用效果、镇痛、镇静和血流动力学的影响。趋向Dex鞘内应用可以弥补传统腰麻的不足，具有广阔的应用前景。Background Dexmedetomidine （Dex） is a highly selective α2 adrenoreceptor （α2AR） agonist acting with analgesia, sedation, anxiolysis and sympatholysis. Animal studies have validated the safety of intrathecal Dex. Objective To review the progress of clinical application of intrathecal Dex and to provide further guidance for its clinical application. Content The article discusses the safety of intrathecal Dex and the influence of local anesthetic effect, antinociption, sedation and hemodynamics. Trend Intrathecal Dex can make up for deficiencies in the traditional spinal block and has a promising future.
