糖肾宁2号方治疗Ⅳ期糖尿病肾病临床疗效及其对氧化应激指标的影响

摘　　要：目的观察糖肾宁2号方对Ⅳ期糖尿病肾病(DN)患者的临床疗效及其对氧化应激指标的影响,探讨其干预DN的作用机理。方法将70例Ⅳ期DN患者随机分为治疗组(35例)和对照组(35例),2组均予西医常规治疗,治疗组加服糖肾宁2号方,对照组加服氯沙坦钾片,疗程均为12周,观察2组临床疗效及治疗前后血清超氧化物歧化酶(SOD)、丙二醛(MDA)、谷胱甘肽过氧化物酶(GSH-Px)、尿8-羟基脱氧脲苷(8-OHdG)等的变化。结果治疗组临床疗效优于对照组(P<0.05)。治疗组蛋白尿、尿白蛋白/肌酐比率下降明显优于对照组(P<0.05),并可明显降低肌酐,提高肾小球滤过率,治疗前后比较差异有统计学意义(P<0.05)。2组治疗前后组内比较,SOD、MDA、GSH-Px差异均有统计学意义(P<0.05),治疗组尿8-OHdG差异有统计学意义(P<0.05)。治疗后组间比较,SOD、MDA、GSH-Px、尿8-OHdG差异均有统计学意义(P<0.05)。结论糖肾宁2号方可改善Ⅳ期DN患者临床证候、24 h尿蛋白定量、肾功能及氧化应激指标,其作用机制可能与调节氧化防御机能有关。Objective To observe the effects of No.2 Tangshenning decoction onⅣstage diabetic nephropathy （DN） in clinical efficacy and oxidative stress indicators, and explore its mechanism against oxidative stress in treating DN. Methods Seventy cases of Ⅳ stage DN were randomly divided into treatment group （35 cases） and control group （35 cases）. Based on conventional treatment, the treatment group was given No.2 Tangshenning decoction and the control group was treated with Cozaar additionally. The course of treatment was 12 weeks. The clinical efficacy and TCM syndrome score, 24-hour urinary protein quantity, renal function, SOD, MDA, GSH-Px, 8-OHdG before and after treatment were observed. Results The therapeutic effect of the treatment group was superior to the control group （P〈0.05）. The treatment group was significantly better than the control group in decreasing the levels of proteinuria and UACR （P〈0.05）. The treatment group decreased Cr and increased GFR, and significant differences were showed between before and after treatment （P〈0.05）. Between before and after treatment, the differences in SOD, MDA and GSH-Px of two groups were statistically significant （P〈0.05）, and in 8-OHdG of the treatment group （P〈0.05）. After treatment, the differences in SOD, MDA, GSH-Px and 8-OHdG were statistically significant between the two groups （P〈0.05）. Conclusion No.2 Tangshenning decoction can improve the patient＇s clinical syndromes, 24-hour urinary protein quantity, renal function and the oxidative stress indicators ofⅣstage DN, and its mechanism may be related to regulating the function of oxidative defense.
