新双源flash-CT评价冠心病合并2型糖尿病患者血管成像特点分析

摘　　要：目的:探讨评价冠心病(CHD)合并2型糖尿病患者冠状动脉CT血管成像(CTA)的特点。方法:筛选2012年6月至2013年6月冠状动脉造影检查确诊为冠心病的患者460例,其中合并2型糖尿病患者220例,无合并糖尿病患者240例,冠状动脉造影后2周内行新双源flash-CT冠脉成像,分析冠心病合并2型糖尿病患者冠状动脉CTA的诊断特点。结果:新双源flash-CT冠状动脉CTA对2型糖尿病并冠心病患者冠状动脉病变的特异性为88.4%。2型糖尿病并CHD患者的冠状动脉钙化较严重,病变支数多,随着钙化积分的增高,特异性有所下降,但冠状动脉血管成像对于介入治疗仍有指导作用。结论:新双源flash-CT冠状动脉CTA是诊断冠心病并2型糖尿病冠状动脉病变可靠、无创的检查方法,但严重钙化者要结合冠脉造影结果进行综合判断。Objective To explore the diagnostic characteristics of the coronary CT angiography （CTA） of coronary heart disease （CHD） combined with type 2 diabetes mellitus. Methods A total of 460 cases diagnosed as CHD by coronary angiography （CAG） admitted from Jun 2012 to Jun 2013 were enrolled in this study, among whom there were 220 ones with type 2 diabetes mellitus and 240 ones without type 2 diabetes mellitus. Dual-source flash-CT angiography was performed two weeks after CAG, and the diagnostic characteristics of the coronary CTA were analyzed. Results The value was 88.4% for the specificity of coronary CTA for type 2 diabetes mellitus combined with CHD. Calcification of coronary arteries was common with large numbers of branches diseased. The specificity decreased when coronary artery calcification score increased, while coronary CTA was still of significance for the interventional therapy. Conclusion Dual-source flash-CT coronary CTA is reliable and non-invasive for the diagnosis of CHD combined with type 2 diabetes mellitus, and CAG has to be involved in the patients with severe calcification.
