双源CT在“胡桃夹”综合征中的诊断价值及应用

摘　　要：目的：探讨双源CT在“胡桃夹”综合征中的临床应用及诊断价值。方法：回顾性分析从2010年1月至2012年11月临床怀疑“胡桃夹”综合征的25例患者资料（其中女性8例，男性17例，年龄为6-49岁），这些患者均采用西门子SOMATOM Definition双源64层CT进行检查，包括肾动脉CTA、肠系膜CTA、肾静脉CTV、肝胆脾胰肾增强CT．对其左肾静脉的受压程度进行显示。结果：螺旋CT平扫：左肾外形较右肾大；CTA及增强扫描：25例均能清晰、直观显示左肾静脉的形态及立体走形，各数据可得到精确测量，所有病例均符合“胡桃夹”综合征CT征象；MPR、MIP、VRT显示：肠系膜上动脉与腹主动脉的夹角变小（〈40°），左肾静脉压力增高。结论：双源CT三维重建技术能清楚地显示肠系膜上动脉和腹主动脉形成的夹角、左肾静脉及毗邻的解剖结构．对临床怀疑“胡桃夹”综合征的患者可作为首选的诊断方法。Objective To explore the clinical application and diagnostic value of dual-source CT on nutcracker syndrome. Methods From January 2010 to November 2012, twenty-five patients （including 8 female patients, 17 male patients, aged from 6 to 49 years） with suspected nutcracker syndrome in our hospital were examined by Siemens SOMATOM Definition dual-source 64-slice CT, involving renal artery CTA, mesentery CTA, renal vein CTV, and enhanced CT of liver, gallbladder, spleen, pancreas and kidney to show the compression extent of the left renal vein, the clinical data of which was retrospectively analyzed. Results The shape of left kidney was larger than right kidney on spiral CT scanning. The shape and three dimensional appearance of left renal vein for the 25 patients were shown clearly and intuitively in enhanced scanning or CTA. The essential data were accurately measured. All patients were in line with nutcracker syndrome on CT characteristics. The results of MPR, MIP, VRT showed that the angle between superior mesenteric artery and abdominal aorta became small （〈40°） and the pressure of left renal vein increased. Conclusion The three dimensional reconstruction technology of dual-source CT can show clearly the angle between superior mesenteric artery and abdominal aorta and the anatomical structures of left vein and adjacent organizations, which can be used as the preferred method of nutcracker syndrome diagnosis.
