摘 要:目的评价化学发光分析(CLIA)检测乙型病毒性肝炎血清标志物(HBV-M)的临床应用价值。方法对随机选取的313例血样用电化学发光免疫分析(ECLIA)和CLIA检测HBV-M,以ECLIA为参照系统,对CLIA进行方法学评价。结果 CLIA检测HBsAg和HBsAb的灵敏度、特异度和总符合率均较高(>95%);而检测HBeAg、HBeAb和HBcAb的总符合率分别为99.68%、83.39%、87.86%,灵敏度分别为83.33%、39.29%、74.48%。结论 CLIA与ECLIA在临床用于HBV-M检测时差异明显,尤其是HBeAb的检测灵敏度较低,需引起注意。Objective To evaluate the clinical value of chemiluminescence immunoassay( CLIA) in detection of hepatitis B virus markers. Methods A total of 313 blood samples were selected randomly to detect the HBV markers using electro-chemiluminescence immunoassay( ECLIA) and CLIA. The performance of CLIA was evaluated by using well-established ECLIA as the " gold standard". Results CLIA showed high senstivity,specificity and total coincidence rates for detecting HBsAg and HBsAb( 95%),while the total coincidence rates of CLIA for detecting HBeAg,HBeAb and HBcAb were 99. 68%, 83. 39%,87. 86% respectively,the senstivities were 83. 33%,39. 29% and 74. 48%,respectively. Conclusion Significant discrepancies were observed in detection of HBV markers using ECLIA and CLIA,especially lower senstivity for HBeA,requiring more attention.
