艾司洛尔在鼻腔鼻内镜手术中的应用

摘　　要：目的：观察艾司洛尔在鼻腔局麻手术中预防肾上腺素β作用对血压升高、心率增快的效果。方法：将择期行鼻腔鼻内镜手术患者（ASAⅠ-Ⅱ）120例随机分为观察组和对照组各60例。观察组在鼻腔填塞浸渍含1/1 000肾上腺素的2%利多卡因液的棉纱条局麻时即静脉注射艾司洛尔0.2 mg/kg,对照组不给药,分别记录鼻腔填塞局麻药棉纱条前、填塞棉纱条后各时点的平均心率（HR）、平均动脉压（MAP）。结果：观察组患者在鼻腔填塞纱布前和后HR、MAP变化无显著性差异（P〉0.05）;对照组患者在局麻后HR、MAP均有显著性差异（P〈0.05）。结论：局麻下行鼻腔手术的患者在鼻腔填塞含肾上腺素的局麻药棉纱条局麻时,静脉注射艾司洛尔可预防因肾上腺素所致的血压增高、心率增快。Objective： To observe the effect of Esmolol in endoscopic sinus surgery with local anesthesia for prevention of epinephrine β action that induced hypertension and increased heart rate. Methods： 120 patients undergoing endoscopic sinus surgery （ASA with level Ⅰ-Ⅱ ） were divided into Group A and Group B randomly. In Group A, the patients had intravenous injection with Esmolol 0.2 mg/kg when adopted nasal packing with cotton yam which contained 1/1 000 epinephrine and 2% Lidocaine for local anesthesia, while in Group B with no Esmolol. Recorded average heart rate and MAP in both groups at each time point before and after nasal packing with cotton yam which contains local anesthetics. Results：Before and after nasal packing with cotton yarn, the average heart rate and MAP in Group A had no significant difference （P 〉 0.05 ）, While there was a significant differences in Group B （ P 〈 O. 05 ）. Conclusion ： When adopted nasal packing with cotton yam which contained epinephrine and local anesthetics for patients of undergoing endoscopic sinus surgery,intravenous injection with Esmolol is a prevention of epinephrine action that induces hypertension and increases heart rate.
