北京海淀区≥40岁居民脑卒中患病现状及其影响因素

摘　　要：目的了解北京市海淀区≥40岁居民脑卒中患病现状及其影响因素,为脑卒中的预防控制提供科学依据。方法采用整群抽样方法对在海淀区5个社区抽取的10 321名≥40岁常住居民进行问卷调查、体格检查和颈部彩超检查。结果北京市海淀区≥40岁居民脑卒中患病率为3.9%,年龄标化患病率为3.5%;397例脑卒中患者中,376例(94.7%)为缺血型脑卒中,17例(4.3%)为出血型脑卒中,4例(1.0%)为缺血型和出血型脑卒中两者兼有;多因素非条件logistic回归分析结果显示,年龄≥70岁、吸烟、高血压、心脏病、血脂异常、糖尿病、明显超重或肥胖、脑卒中家族史为海淀区≥40岁居民脑卒中患病的危险因素,职员和其他职业为海淀区≥40岁居民脑卒中患病的保护因素。结论北京市海淀区≥40岁居民脑卒中患病率较高,以缺血型为主;年龄、职业、吸烟、高血压、心脏病、血脂异常、糖尿病、明显超重或肥胖、脑卒中家族史是海淀区≥40岁居民脑卒中患病的主要影响因素。Objective To examine stroke prevalence and its risk factors among population aged 40 years old and over in Haidian district, Beijing. Methods Totally 10 321 subjects were selected with cluster sampling method. Each participant was invited to receive a face-to-face standardized questionnaire interview,physical examination and carotid ul- trasonography. Results Stroke prevalence was 3.9% and age standardized prevalence was 3.5% in the participants. Of 397 stroke patients, 376 （94. 7 % ） had ischemic stroke, 17 （4. 3 % ） had hemorrhagic stroke, and 4 （ 1.0 % ） had both. Un- conditioned logistic regression analysis showed that age（/〉70 years old）, smoking,hypertension,heart disease,lipid ab- normality, diabetes, overweight, and family history of stroke were major stroke risk factors for the residents aged 40 years old and over in Haidian district. Some occupations （clerk, students/freelancers）were protective factors of stroke for the population. Conclusion People aged 40 years old and over in Haidian district had a high prevalence of stoke, and the main type was ischemic stroke. Age, occupation, smoking, hypertension, heart disease, lipid abnormality, diabetes, over- weight, and familial history of stroke are major risk factors for stroke in the residents aged 40 years old and over in Haidi- an district.
