锁定钢板与普通钢板治疗老年移位肱骨近端骨折的效果观察

摘  要:目的观察锁定钢板与普通钢板治疗老年移位肱骨近端骨折的效果。方法将42例老年移位肱骨近端骨折患者随机分为两组,各21例。观察组采用锁定钢板治疗,对照组采用普通钢板治疗。观察两组手术时间、术中出血量及术后3个月、6个月、12个月的Neer评分情况,同时比较两组肩关节活动范围、术后并发症等。结果观察组手术时间短于对照组(P<0.01),术中出血量少于对照组(P<0.01)。两组患者术后6个月Neer评分均高于术后3个月(P<0.01),术后12个月Neer评分均高于术后6个月(P<0.01);术后同期Neer评分比较,观察组均高于对照组(P<0.01)。观察组患者上举和内旋活动角度均大于对照组(P<0.01)。结论锁定钢板治疗老年移位肱骨近端骨折临床疗效显著,值得临床推广应用。Objective To explore the clinical effect of locking plate and ordinary steel plate for the treatment of displaced proximal humeral fracture. Methods Forty-two cases with displaced proximal humeral fracture were randomly divided into two groups (n = 21, each). The observation group was treated with locking plate, while the control group was treated with traditional ordinary steel plate. Operation time and bleeding volume during operation ,and Neer scores of postoperative 3 months ,6 months, 12 months were compared. Shoulder range of motion, and postoperative complications were observed. Results Operation time, intraoperative blood loss of observation group were lower than those of the control group( P 〈 0.01 ). Neer scores postoperative 6 months was higher than that of postoperative 3 months in the two groups and the difference was statistically significant ( P 〈 0.01 ), Neer scores of postoperative 12 months was higher than that of 6 months( P 〈 0.01 ). Neer scores of the same time period post-operative of the observation group was higher than that of control group( P 〈 0.01 ). The angle of shoulder joint range of motion of the observation group was bigger than that of the control group( P 〈 0.01 ). Conclusion Locking plate in treatment of proximal humeral fracture has obvious curative effect, and is clinical worthy.
