手术治疗31例肱骨远端C型骨折  

Surgical treatment for 31 cases of C type fracture of distal humerus

摘　　要：目的 探讨肱骨远端C型骨折的手术方法及疗效。方法手术治疗肱骨远端C型骨折3l例：肱三头肌舌形瓣人路12例，尺骨鹰嘴V形截骨人路19例；采用双钢板内固定18例，钢板加张力带内固定13例。结果 患者均获得随访，时间6～18个月。按Aitken和Rorabeek标准进行功能评定：优9例，良12例，可6例，差4例。尺骨鹰嘴V形截骨人路组功能评定优于肱三头肌舌形瓣人路组（P〈0．05）；双钢板内固定功能评定与钢板加张力带内固定差异无统计学意义（P〉0．05）。结论 尺骨鹰嘴V形截骨人路暴露充分，可以早期功能锻炼，利于截骨部骨折的愈合及肘关节功能恢复。两种内固定功能评定无明显差异，应根据骨折类型选用不同的内固定方法。Objective To evaluate the surgical management and its results of C type fracture of distal end of hume-rus. Methods 31 patients with C type fracture were treated, 19 patients of which were treated surgically through ap-proach of V shape osteotolny of olecranon, 12 patients of which were treated surgically through approach of transtri-ceps brachial sides. 13 patients were treated with band and plating internal fixation, 18 patients were treated with double plating internal fixation. Results 31 patients were followed up for 6 ～ 18 months. The function of the elbow according to Aitken and Rorabeek scale showed excellent in 9 cases, good in 12, fair in 6 and poor in 4. The patients were treated surgically through approach of V-shape osteotolny of olecranon winned the better effect than the patients treated surgically through approach of trsnstriceps brachiul sides （P 〈 0. 05）. However, both the patients in the two groups were nulogous in the function of the elbow （P 〉 0. 05）. Conclusions The technique for the treatment of type C fracture of distal end of hmnerus through trans-olecranon approach has many advantages, such as sufficient expo-sure, easy to fracture cure and earlier exercise. There is no different in the treatment of type C fracture of distal end of hmnerus with wire tension band and plate internal fixation, or with double plating internal fixation. Different frac-ture type should be treated by different internal fixation methods.
