摘 要:目的探讨缺铁性贫血与老年冠心病的发生及严重程度的相关性。方法对入选的冠心病患者测定各项临床指标,根据冠脉造影、血红蛋白等结果分为缺铁性贫血组与非贫血组及单支病变组、多支病变组等亚组,分别进行对照研究。结果 (1)缺铁性贫血组和非贫血组性别、年龄、体重、血尿酸、胆固醇、甘油三酯、糖尿病、高血压病等病史在两组间差异无统计学意义(P>0.05)。(2)缺铁性贫血与冠状动脉病变的关系:对于轻度狭窄病变及中度狭窄病变,缺铁性贫血组所占比例显著低于非贫血组(P<0.05);对于双支病变、三支病变与重度狭窄及完全闭塞病变,缺铁性贫血组所占比例显著高于非贫血组(P<0.05);(3)Logistic回归分析显示年龄、糖尿病和缺铁性贫血是冠心病发病的危险因素。结论在冠心病老年患者,缺铁性贫血提示病变较重的冠心病类型,故缺铁性贫血可能是冠心病的一个危险因素。Objective To evaluate the association between the iron deficiency anemia and thepresence and severity of angiographic coronary artery disease (CAD). Method Select patients that underwent coronary an- giography (CAG) for study object. According to the results of the CAG, the hemoglobin and the bone marow,most cases were divided into control group that dose not have had anemia disease and the iron deficiency anemia group that has not only coronary heart disease but also the iron deficiency amemia. According to the scope and count of the sick coronary artery, all cases were divided into single- vessel group and multi - lesion group of two subgroup. Application of t - test, chi - square test of all the data for processing. Results (1) The iron deficiency anemia group and the control group do not have statistical difference comparised on clinical features such as gen- der, age, weight , uric acid, cholesterol, triglycerides, diabetes and hypertension( P 〉 0.05 ). (2)The relationship between the iron deficiency anemia and coronary artery disease: as for mild stenosis, there is not significant differ- ence between the two groups(P 〈 0.05) ;as for double - vessel, multiple - vessel subgroup, severe stenosis and total occlusion subgroup, the iron deficiency anemia group were significantly higher than the proportion of the control group( P 〈 0.05). (3) Logistic analysis show Age, Gender and the iron deficiency anemia were risk factors of CHD. Conclusion The iron deficiency anemia may indicate the heavier type of the co ronary heart disease. Therefore, the iron deficiency anemia may be a risk factor for coronary heart disease.
