小剂量布比卡因腰-硬联合麻醉在高龄患者股骨颈骨折手术中的应用

摘　　要：目的：探讨小剂量布比卡因腰-硬联合麻醉用于高龄患者股骨颈骨折手术的效果。方法选择ASAⅡ～Ⅲ级高龄患者股骨颈骨折择期手术患者58例，采用0.75％布比卡因6 mg腰-硬联合麻醉后，观察手术中的麻醉效果、术中呼吸、血流动力学的变化以及麻醉后并发症发生等情况。结果术前和术中呼吸、血流动力学无明显改变（ P均＞0.05）。结论小剂量布比卡因腰-硬联合麻醉在高龄患者股骨颈骨折手术中的应用，对患者呼吸、循环系统抑制作用轻微，具有效果确切、安全可靠、不良反应小等优点。Objective To investigate the anesthesia effect of small dose bupivacaine combined spinal -epi-dural anesthesia in the surgical treatment of femoral neck fracture in elderly patients .Methods Fifty-eight ASAⅡ~Ⅲgrade elderly patients undergoing elective femoral neck fracture surgery received small dose （0.75%bupivacaine 6 mg） spinal-epidural anesthesia , and the anesthetic effect , intraoperative respiratory , hemodynamic changes , and the com-plications caused by anesthesia were observed .Results There were no significant changes in preoperative , intraoper-ative respiratory , or hemodynamic indexes （ P all 〉0.05 ） .Conclusion Small dose bupivacaine combined spinal-epidural anesthesia for femoral neck fracture surgery in elderly patients has less inhibition effect on respiratory and circulatory system, with the advantages of exact anesthesia effect , higher safety, less adverse reactions , etc.
