摘 要:目的初步评价利奈唑胺注射液治疗颅内感染的临床疗效与安全性,以更好的指导临床药物应用,使颅内感染患者得到合理规范化治疗。方法回顾性分析2009年3月-2012年11月应用利奈唑胺治疗的25例神经外科疾病术后并发颅内感染患者临床资料,并总结国内外相关文献报道;采用法国生物梅里埃公司VITEK-2Compact全自动细菌鉴定及药敏分析系统进行药敏试验。结果 25例患者应用利奈唑胺治疗后的总体治愈率为100.0%,患者头痛、恶心、呕吐等症状缓解率为100.0%,体温、血常规、脑膜刺激征缓解率为100.0%,革兰阳性球菌清除率为100.0%,脑脊液常规缓解率为91.3%,2例患者出院时脑脊液常规白细胞仍高于正常,但无其他颅内感染迹象,全部患者应用利奈唑胺均未见不良反应。结论利奈唑胺可有效治疗革兰阳性球菌所致颅内感染,患者临床症状缓解情况良好,疗效明确,临床安全性好。OBJECTIVE To evaluate the clinical efficacy of linezolid injection in treatment of intracranial infections and observe the safety so as to guide the clinical application and achieve standardized treatment of intracranial infections.METHODS From Mar 2009to Nov 2012,totally 25patients complicated with postoperative intracranial infections who were treated with linezolid in the department of neurosurgery were enrolled in the study,then the clinical data of the subjects were retrospectively analyzed,the relevant reports in literature at home and abroad were summarized,the identification of strains was carried out by using VITEK-2Compact automatic bacterial identification system of BioMérieux,France;the drug susceptibility testing was performed with the use of drug susceptibility analysis system.RESULTS Of the 25patients who were treated with linezolid,the total cure rate was 100.0%,the recovery rate of the symptoms such as headache,nausea,and vomiting was 100.0%,the recovery rates of the body temperature,blood routine examinations,and signs of meningeal irritation were 100.0%;the clearance rate of gram-positive cocci was 100.0%;the recovery rte of cerebral spinal fluid routine was 91.3%;the level of white blood cells in the cerebral spinal fluid routine of two patients remained higher than normal,however, there was no other sign of intracranial infection.The adverse reactions did not occur in any of the patients receiving the treatment with linezolid.CONCLUSIONLinezolid is effective in treatment of the intracranial infections caused by the gram-positive cocci with good recovery of clinical symptoms,with the clinical efficacy definite,the clinical safety high.
