综合护理辅助机械通气治疗危重症手足口病的效果

摘　　要：目的探讨综合护理辅助机械通气治疗危重症手足口病的效果。方法将本院收治的50例危重症手足口病患儿随机分为观察组25例和对照组25例。两组均行相同的临床治疗方案,在机械通气治疗中,对照组采用常规护理,观察组采用综合护理。比较两组的临床疗效、血气分析指标、机械通气时间、胸部X线片改善时间及住院时间。结果观察组的痊愈率为92%,死亡率为8%;对照组的痊愈率为84%,死亡率为16%,差异有统计学意义(P<0.05)。观察组治疗及护理后的PaO2高于对照组,PaCO2和HCO3-低于对照组,差异有统计学意义(P<0.05)。观察组的机械通气时间、胸部X线片改善时间及住院时间均短于对照组,差异有统计学意义(P<0.05)。结论及早提供机械通气呼吸支持及护理干预,能提高危重症手足口病患儿的临床疗效,同时提高救治成功率,降低病死率。Objective To study effect of comprehensive nursing care assisted mechanical ventilation in the treatment of severe hand,foot and mouth disease. Methods 50 cases of children with hand,foot and mouth disease in our hospital were selected and randomly divided into observation group（25 cases）and control group（25 cases）.Same clinical treatment was used in two groups.In mechanical ventilation therapy,routine nursing care was used in control group,comprehensive nursing care was used in observation group.Clinical efficacy,blood gas analysis,mechanical ventilation,chest X-ray to improve time and length of stay in two groups was compared respectively. Results Cure rate of observation group was 92% ,the mortality rate of observation group was 8% ,cure rate of control group was 84% ,the mortality rate of control group was 16%,with statistical difference（P〈0.05）.After the treatment and nursing,PaO2 of observation group was higher than that of control group,PaCO2,HCO3^-of observation group was lower than that of control group respectively,with statistical difference （P〈0.05）.Mechanical ventilation,chest X-ray to improve time and length of stay in observation group was shorter than that in control group respectively,with statistical difference （P〈0.05）. Conclusion Respiratory support as soon as possible to provide mechanical ventilation and nursing intervention treatment,can improve the effects of children with severe hand,foot and mouth disease,improve treatment success rate and reduce the fatality rate.
