12所综合性医院护士识别压疮分期准确率情况分析

摘　　要：[目的]分析12所综合性医院护士识别压疮分期的准确率及存在问题,并提出对策.[方法]对462名来自12所综合性医院的压疮护理小组护士采用培训包集中讲解和讨论2h,发放20张共23处压疮不同分期的照片,看照片答题,在1h内完成考核,使用统-的评分标准评卷并复核,分析压疮分期判断准确率和存在问题.[结果]462名护士在规定时间内完成答题,护士判断压疮分期的总准确率为92.89% （87.45% ～97.62%）;存在的主要问题：不能准确识别Ⅲ期和难以分期压疮,误判率较高,分别为12.34% 和12.55%.得分64 分～100分（94.85分±3.81分）,≥90分者457人,占98.92%.三级医院护士得分（95.15分±4.07分）高于二级医院护士（94.00分±2.24分）.不同护龄、学历和职称护士得分比较差异有统计学意义（P〉0.05）.[结论]使用培训包集中讲解和讨论及采用压疮照片识别分期形式的考核有助于提高护士识别压疮的准确率,但对Ⅲ期压疮和难以分期压疮误判率较高,需要进一步加强岗位培训和指导.Objective:To analyze accuracy rate of nurses' recognition of pressure ulcers in 12general hospitals and the existing problems,and put forward countermeasures.Methods:A total of 462nurses in pressure ulcers care team in 12general hospitals were selected,and they received training kit explanation and discussion for 2h,issuing 20pictures with a total of 23 places of different stages of pressure ulcers,answered the questions through looking at photos,completed the assessment within 1h,used unified criteria for evaluation and review,analyzed the accuracy rate and problems of staging judgment of pressure ulcers.Results:All 462nurses answered the problems within the prescribed time,and the overall accuracy rate was 92.89%(87.45%~97.62%)of nurse determining stage of pressure ulcers;the main problems:nurses cannot accurately identifyⅢ period of pressure ulcers and pressure ulcers which was difficult to stage,and their misjudgment rate was higher,12.34% and 12.55% respectively.They scored 64points to 100points(94.85±3.81),more than 90in 457people, accounting for 98.92%.Score of nurse in three grade hospitals(95.15±4.07) was higher than that of nurses in two grade hospitals(94.00±2.24). There was statistically significant difference in score among different care age,education and nursing titles of nurses(P>0.05).Conclusion:Presentations and discussions focused on training packages and the use of pressure ulcers photo identification stage assessment is conductive to improve nurses' recognition accuracy rate of pressure ulcers,but misjudgment rate forⅢstage pressure ulcers and difficult stage of pressure ulcers is higher, it need to further strengthen job training and guidance.
