腹、盆部Castleman病的CT、MRI表现与病理对照

摘　　要：目的:探讨腹、盆部Castleman病的CT、MRI特征,并与病理学对照。方法:回顾性分析13例经手术病理证实CD的CT、MRI及病理资料。结果:①局限型12例,病理类型均为透明血管型,弥漫型1例为浆细胞型。11例肿块边缘光整,与周围组织边界清晰,2例包绕腹主动脉。②CT平扫:12例呈等密度,密度均匀,2例中心见分支状钙化;1例为稍低密度,见囊变坏死。CT增强:9例动脉期呈不均匀显著强化,3例中度均匀强化,1例轻度均匀强化,5例周缘见明显强化血管影;静脉期9例持续均匀强化,3例中心填充强化,1例环状强化;延迟期均呈较均匀稍高密度。5例行MRI检查,平扫T1WI 3例呈稍低信号,2例呈等信号,T2WI及DWI均呈高信号,信号均匀,边界清晰;增强扫描与CT强化方式一致。结论:腹、盆部CD的CT、MRI表现与病理类型密切相关,尤其是透明血管型CD表现具有一定特征,术前影像学检查,尤其是MRI检查,对CD诊治具有重要价值。Purpose： To investigate the CT, MRI features and compared with pathology of Castleman disease （CD）. Methods： CT, MRI findings and pathology data of 13 cases with CD proved by pathology were analyzed retrospectively. Results： （1） 12 cases were localized and pathological types were hyaline vascular type, 1 case was diffuse and plasma cell type. 11 cases margin were clear boundaries with the surrounding tissue, 2 case surrounding the abdominal aorta. （2） CT scan showed that 12 cases were equal density and 2 cases were seen branch calcification at centra, 1 case was slightly low density and with cystic necrosis.Enhanced CT： 9 cases showed heterogeneous significantly enhanced 3 cases homogeneous moderate enhancement and 1 case mild homogeneous enhancement at arterial phase, 5 cases were seen significantly enhanced tumor blood vessels around the edge or shadow; 9 cases continued homogeneous enhancement at intravenous, 3 cases center fill strengthen, 1 case showed ring enhancement; all showed slightly more uniform density at delay period. Among 5 cases of MRI examination, 3 cases showed slightly lower signal and 2 cases showed equal signal on T1WI, all showed high signal on T2WI and DWI, with uniformity signal and clear boundaries. And MRI enhanced modes were consistent with CT enhanced. Conclusion： CT, MRI performances of CD of abdomen and pelvis were closely related to the histological type, especially hyaline vascular type CD performance had certain characteristic, Preoperative imaging examinations especially MRI had great value for the diagnosis of CD.
