高频超声在维医沙疗治疗类风湿性关节炎中的应用价值

摘　　要：目的探讨高频超声在维医沙疗治疗类风湿性关节炎(rheumatoid arthritis,RA)中的应用价值。方法行维医沙疗的RA膝关节病变患者56例(RA组),采用高频超声观察治疗前、后髌上囊液体厚度、髌上囊滑膜厚度、股骨内髁及外髁软骨厚度、髌上囊滑膜多普勒彩色血流分级等改变,并与56例体检健康者(对照组)进行比较。结果沙疗前RA组髌上囊液体厚度、髌上囊滑膜厚度较对照组增加,股骨内踝、外踝软骨厚度较对照组减少(P<0.05);RA组沙疗后髌上囊液体厚度、髌上囊滑膜厚度及髌上囊滑膜彩色血流分级与沙疗前比较差异有统计学意义(P<0.05),股骨内踝及外踝软骨厚度与沙辽前比较差异无统计学意义(P>0.05)。结论高频超声检查是诊断RA的可靠方法,可作为维医沙疗有效性的判定标准。Objective To investigate the application of high-frequency ultrasound in sand therapy of Uygur medicine for rheumatoid arthritis （RA）. Methods Fifty-six RA patients （RA group） with the knee lesions receiving sand therapy of Uygur medicine were examined the changes of suprapatellar bursa fluid thickness, synovium thickness, medial femoral condyle and lateral condyle cartilage thickness, and color flow grading of suprapatellar bursa synovium by high-frequency ultrasound before and after treatment, and were compared with another 56 healthy volunteers （control group）. Results The thickness of suprapatellar bursa fluid and synovium increased, and medial femoral condyle and lateral condyle cartilage thickness decreased in RA group in comparison with control group before treatment （P〈0.05）. There were significant differences in the thickness of suprapatellar bursa fluid and synovium, and color flow grading of suprapatellar bursa synovium after treatment in comparison with before treatment in RA group （P〈0.05）, and there was no significant difference in medial femoral condyle and lateral condyle cartilage thickness after treatment in RA group （P〉0.05）. Conclusions High-frequency ultrasound is a reliable screening method for diagnosing RA, and can be an effective criteria for evaluating the therapeutic effect of sand therapy of Uygur medicine.
