单核细胞趋化蛋白-1启动子-2518 A/G位点基因多态性与狼疮性肾炎关联性的Meta分析

摘　　要：目的探讨单核细胞趋化蛋白-1(MCP-1)启动子-2518 A/G位点基因多态性与狼疮性肾炎(LN)的关系。方法通过检索PubMed、Embase、The web of science、中国学术期刊网全文数据库(CNKI)、万方科技期刊全文数据库、中国生物医学数据库(CBM),收集所有MCP-1启动子-2518 A/G位点基因多态性与LN易患性的病例对照研究,按纳入排除标准并进行质量评价后,提取有效数据进行Meta分析。结果共纳入11项研究,其中共包括LN患者989名,健康对照者1 935名,采用共显性模型对结果进行分析。GG/AA、GG/AG、AG/AA三组基因型合并后的OR值分别为1.60〔95%CI(1.00,2.59),P=0.052〕、1.34〔95%CI(1.06,1.70),P=0.015〕、1.28〔95%CI(0.94,1.94),P=0.120〕,等位基因G/A合并后的OR值是1.32〔95%CI(1.04,1.66),P=0.020〕,其中GG/AG、G/A组差异有统计学意义。亚组分析显示,在美洲人群中,MCP-1启动子-2518 A/G位点基因多态性与LN存在很强的相关性,而在亚洲及欧洲人群中,还没有发现二者的相关性。结论在美洲人群中,MCP-1启动子-2518 A/G位点基因多态性与LN的发生相关,而在亚洲和欧洲人群中,尚不能得出同样的结论。Objective The purpose of this study is to explore whether the monocyte chemoattractant protein -1 （MCP-1） promoter -2518 A/G polymorphism confers susceptibility to lupus nephritis （LN） .Methods Literature search of PubMed, Embase, Web of Science, CNKI, Wanfang and CBM databases was conducted on studies about the correlation be -tween MCP-1 promoter -2518 A/G gene polymorphisms and lupus nephritis.All the cases involved were brought into control study and after quality evaluation the effective data were extracted for Meta analysis.Results Eleven studies （ including 989 LN patients and 1 935 controls ） were included in our meta -analysis.Co -dominant model was used to analyze the results.The pooled OR （95%CI） of GG/AA, GG/AG, AG/AA were 1.60 〔95%CI （1.00, 2.59）, P =0.052〕, 1.34 〔95%CI （1.06, 1.70）, P=0.015〕, 1.28 〔95%CI （0.94, 1.94）, P=0.120〕 and the pooled OR （95%CI） of G/A was 1.32〔95%CI （1.04, 1.66）, P=0.020〕 individually, statistical differences were existed in GG/AG and G/A.Subgroup analysis indicated MCP-1 promoter -2518 A/G polymorphism had strong correlation with lupus nephritis in the people of Americas, but no similar correlation in the people of Asia and Europe .Conclusion The author suggests among American people , MCP-1 pro-moter -2518 A/G polymorphism may play a role in the occurrence of lupus nephritis , however it is not true of Asian and Europe-an population.
