轻度慢性乙型肝炎中医证候与肝组织病理的相关性分析

摘　　要：目的:探讨轻度慢性乙型肝炎(CHB)中医证候分布特点,并分析其证候与肝组织病理之间的相关性。方法:对收集的82例轻度CHB的患者进行中医辨证分型,并行肝组织病理活检,分析中医证候与肝组织病理之间的相关性。结果:经过聚类分析,82例轻度CHB中医证候可归为4型:肝气郁结证,肝郁脾虚证,肝肾阴虚证,肝胆湿热证;以肝气郁结证和肝郁脾虚证为主,所占比例为74.39%;肝组织学炎症以G2为主,纤维化程度以S1、S2为主,经χ2检验,P<0.05。结论:轻度CHB的4类中医证型符合CHB中医证候临床特点。肝组织学炎症及纤维化程度以轻中度为主,且CHB的肝组织病理改变与中医证候有一定相关性。Objective： To investigate TCM syndromes distribution characteristics of mild chronic hepatitis B （ ALT 1 - 2 × ULN）, and analyze the correlation between TCM syndromes and liver tissue pathology. Methods： The researchers collected 82 cases of mild chronic hepatitis B patients （ALT 1 - 2 × ULN）, performed their TCM syndromes typing, and liver biopsy, analysed the correlation between TCM syndromes and liver tissue pathology. Results： After clustering analysis, 82 cases of mild chronic hepatitis B were classified into 4 types ： syndrome of stagnation of liver Qi, syndrome of stagnation of liver Qi and spleen deficiency, syndrome of Yin deficiency of liver and kidndy, syndrome of dampness-heat of liver and gallbladder. Syndrome of stagnation of liver Qi, syndrome of stagnation of liver Qi and spleen deficiency accounted for the major part, 74. 39%. Liver histological inflammation was dominated by G2, fibrosis degree by S1 and S2, by cbi-square test, P 〈 0.05. Conclusion： The four TCM syndromes of mild chronic hepatitis B meet the clinical features of TCM syndromes of chronic hepatitis B. Liver tissue in- flammation and fibrosis degree is mainly mild to moderate. Chronic hepatitis B liver tissue pathology and TCM syndromes have some relevance.
