纤维支气管镜肺泡灌洗治疗社区获得性卒中相关性肺炎疗效观察

摘　　要：目的观察纤维支气管镜肺泡灌洗治疗社区获得性卒中相关性肺炎(SAP)患者的临床疗效。方法将66例社区获得性SAP患者随机分为对照组和观察组,每组33例。对照组患者采用内科综合治疗,观察组患者在内科综合治疗的基础上给予纤维支气管镜肺泡灌洗治疗。观察2组患者的发热持续时间、治疗前及治疗24、48、72 h和7 d后的血气分析、啰音消失时间、气管切开例数、住院时间及住院费用。结果治疗结束后,观察组患者发热持续时间、啰音消失时间、气管切开例数、住院时间及住院费用均显著低于对照组(P<0.05,P<0.01)。治疗24、48、72 h及7 d后,观察组患者动脉血氧分压高于对照组,动脉血二氧化碳分压低于对照组,差异均有统计学意义(P<0.01)。结论纤维支气管镜肺泡灌洗是治疗社区获得性SAP有效的辅助治疗措施。Objective To observe the effect of fibrous bronchoscope bronchial alveolar lavage on community acquired stroke associated pneumonia （SAP）. Methods Sixty-six patients with community acquired SAP were randomly divided into control group（33 cases） and observation group（33 cases）. All the patients were given the same regular medical treatment. In addition, the patients in observation group were treated with fibrous bronchoscope bronchial alveolar lavage. Duration of fever, tales disappearing,blood gas analysis, the number of tracheal incision, hospitalization days and hospitalization expenses were compared between the two groups before and 24,48,72 hours and 7 days after treatment. Results Duration of fever, rales dis- appearing, the number of tracheal incision, hospitalization days and hospitalization expenses in observation group were signifi- cantly lower than those in control group after treatment（ P 〈 0.05 ,P 〈 0.01 ）. At 24,48,72 hours and 7 days after treatment, the arterial partial pressure of oxygen was higher while the arterial carbon dioxide tension was lower in observation group than those in control group, there were significant differences between the two groups （ P 〈 0.01 ）. Conclusion The method of fi- brous bronchoscope bronchial alveolar lavage is an effective way for SAP.
