摘  要:目的:研究过敏性紫癜(HSP)患者血中sP-选择素、血管假性血友病因子(vWF)和可溶性细胞间黏附分子1(sICAM-1)水平的变化,探讨其在HSP发病机制中的作用及临床意义。方法:酶联免疫吸附法(ELISA)检测观察组108例HSP患者血清sICAM-1、血浆sP-选择素和vWF,并与对照组30例健康志愿者对照;比较不同分型HSP患者上述指标的差异。结果:观察组血中sP-选择素、vWF和sICAM-1明显高于对照组,差异有统计学意义(P<0.05);108例HSP患者中,肾型组vWF明显高于非肾型组,差异有统计学意义(P<0.05);其他分型组间比较,差异均无统计学意义(P>0.05)。结论:HSP患者血浆sP-选择素、vWF以及血清sICAM-1的高表达,一定程度上反应了患者血小板活化程度和血管内皮受损程度,是HSP两个重要发病机制。vWF能够一定程度反映脏器受损程度。Objective. To observe changes of blood sP-seleetion, yon Willebrand Factor, Soluble intercellular adhesion molecule 1 in allergic purpura (HSP) patients and to explore the role of the HSP in the pathogenesis of HSP as well as its elinical significance. Methods: Method of enzyme-linked immunosorbent (ELISA) was used to detect serum slCAM 1, sP - selection and vWF of 108 HSP patients in the observation group, and the results were compared with 30 volunteers in the control group. Results: The blood sP-selection, vWF and slCAM 1 of the observation group were significantly higher than those of the control group (P〈0.05) ; Among the 108 patients with HSP, patients with renal type had a significant higher vWF than that of non-renal group (P〈0.05); no significant difference was found between other type pf HSP patients (P〉0.05). Oonclusion: Increased expression levels of HSP plasma sP-selection element, vWF and sICAM 1 reflect the degree of platelet activation and endothelial damage, which are two major Dathogenesis of HSP occurrence. VWF can reflect the viscera damages to some extent.
