摘 要:目的探究miR-21和miR-133a在缺血性心肌病(ICM)患者血浆中的表达水平及临床价值。方法选择2012年9月—2013年5月在郑州大学第一附属医院心内科住院治疗的ICM患者48例为ICM组,同时选取该院30例健康志愿者为对照组。收集两组一般临床资料(性别、年龄、吸烟史、是否患有糖尿病等)及相关指标〔总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)及N末端B型脑钠肽原(NT-proBNP)、左心室舒张末期容积(LVEDV)、左心室射血分数(LVEF)〕,运用实时荧光定量聚合酶链式反应法(qRT-PCR)测定两组受试者血浆miR-21和miR-133a的表达水平,分析血浆miR-21和miR-133a水平与有关因素的相关性,评估血浆miR-21和miR-133a在ICM临床诊疗中的价值。结果 ICM组与对照组的性别、年龄、吸烟与否、是否患有糖尿病比较,差异均无统计学意义(P>0.05);而在是否行经皮冠状动冠介入术(PCI)方面比较,差异有统计学意义(P<0.05)。ICM组LDL-C、HDL-C及LVEF低于对照组,TG、NT-proBNP及LVEDV高于对照组(P<0.05);两组舒张压、收缩压及TC比较,差异均无统计学意义(P>0.05)。与对照组〔(4.22±1.78)、(1.37±2.84)〕相比,ICM组血浆miR-21和miR-133a水平〔(8.10±2.93)、(3.28±1.62)〕升高(P<0.05)。ICM组血浆miR-21水平与NT-proBNP和LVEDV呈正相关(r=0.342、0.317,P<0.05),而ICM患者血浆miR-133a水平与年龄、LVEDV、LVEF、TC、TG、LDL-C、HDL-C及NT-proBNP均无直线相关性(P>0.05);且经过偏相关分析(调整年龄、性别、吸烟史、糖尿病、高血压及NT-proBNP水平等因素)后,ICM组中血浆miR-21水平仍与LVEDV呈正相关关系(r=0.334,P<0.05)。结论 ICM患者血浆miR-21和miR-133a表达水平升高,血浆miR-21水平与NT-proBNP及LVEDV呈正相关,故血浆miR-21可能参与了ICM患者的心室重构,可作为ICM诊断及进行风险评估的新的生物标志物。Objective To investigate the expression of plasma miR - 21 and miR - 133a in patients with ischemic cardiomyopathy (ICM) and its clinical significance. Methods General clinical data (gender. age, history of smoking, with diabetes or not, etc) and data of total cholesterol (TC), triglyceride (TG), low density lipoprotein cholesterol (LDL - C) , high density lipoprotein cholesterol (HDL - C), N - terminal B - type brain natriuretic peptide (NT - proBNP), left ventricular end - diastolic volume (LVEDV), left ventricular ejection fraction (LVEF) were collected in 48 ICM patients (ICM group) treated in the First Affiliated Hospital of Zhengzhou University and 30 healthy subjects (control group) from September 2012 to May 2013. Results There was no significant difference in gender, age, smoking history, suffering from diabetes or not between 2 groups (P 〉 0. 05), but there was in PCI performance or not (P 〈 0. 05 ). The levels of LDL - C, HDL - C, LVEF were lower, TG, NT - proBNP, LVEDV were higher in group ICM than in control group, the difference was significant (P 〈 0. 05), but there was no difference in DBP, SBP, TC between 2 groups (P 〉 0. 05 ). The plasma levels of miR - 21 and miR -133a were higher in group ICM[ ( 8. 10 ± 2. 93), (3. 28 ± 1. 62), respectively ] than in control group [(4. 22 ± 1. 78) , (1. 37 ± 2.84), respectively ] (P 〈 0.05). Plasma miR - 21 was positively correlated with NT - proBNP, LVEDV in group ICM (r=0.342, 0.317, P〈0.05), and plasma miR-133a not correlated with age, LVEDV, LVEF, TC, TG, LDL-C, HDL - C or NT - proBNP (P 〉 0. 05). After partial correlation analysis (adjustment of age, gender, smoking, diabetes, hypertension, NT - proBNP and other factors), miR - 21 was still positively correlated with LVEDV in group ICM group (r = 0. 334, P 〈 0. 05).
