摘 要:目的:探讨尿α1、β2微球蛋白在2~4期慢性肾病患者中诊断价值。方法解放军总医院2011年12月至2013年5月肾内科的134例住院患者和50例健康体检者的尿α1、β2微球蛋白和其他肾功能指标进行检测分析,采用MDRD公式进行肾小球滤过率计算。结果慢性肾病组尿α1、β2微球蛋白水平明显高于健康对照组,差异有统计学意义(P<0.01)。2~4期慢性肾病患者中尿α1、β2微球蛋白结果呈明显升高趋势,其差异有统计学意义(P<0.05)。尿α1、β2微球蛋白在3a和3b期中结果有明显差异,3a期尿α1微球蛋白(20.00±18.9)mg/L、3b期(40.5±28.5)mg/L ;3a期β2微球蛋白(4.1±4.0)mg/L、3b期(10.0±8.7)mg/L。相关分析显示,尿α1、β2微球蛋白与肌酐呈显著正相关,与eGFR呈显著负相关。结论尿α1、β2微球蛋白是用于评价慢性肾脏疾病肾功能较好的实验室指标。Objective To explore the value of urine α1 microglobumin and β2 microglobumin for the diagnosis of stage 2-4 chronic kidney disease (CKD) .Methods Urineα1 microglobumin ,β2 microglobumin and the other indi-cator related to kidney function were analyzed in 134 patients with CKD ( CKD group) and 50 healthy subjects (con-trol group) from December 2011 to May 2013 .The estimated glomerular filtration rate (eGFR) was calculated by u-sing the modified Modification of Diet in Renal Disease (MDRD) equation .Results Levels of urineα1 microglobumin andβ2 microglobumin in CKD group were significantly higher than those in control group (P<0 .01) .Levels of urineα1 microglobumin andβ2 microglobumin were increased with the elevation of disease stages of CKD from stage 2 to stage 4 .Levels of urine α1 microglobumin were (20 .00 ± 18 .9) mg/L in stage 3a and (40 .5 ± 28 .5)mg/L in stage 3b ,and levels of urine β2 microglobumin were (4 .1 ± 4 .0)mg/L in stage 3a and (10 .0 ± 8 .7)mg/L in stage 3b ,the differences between the two groups were significant .Correlation analysis indicated that levels of urine α1 microglobu-min andβ2 microglobumin were positively correlated with to creatinine ,negatively correlated with eGFR .Conclusion Urineα1 microglobumin andβ2 microglobumin might be useful indicators for the assessment of renal damage in pa-tients with CKD .
