摘 要:目的对比超声光散射成像系统、超声弹性成像技术及二者联合对乳腺病灶鉴别诊断价值的研究。方法回顾经穿刺活检及手术病理证实的96例患者共116个乳腺病变,分析其超声光散射断层成像系统图像及超声弹性成像图像,并与术后病理及穿刺活检结果对照。结果 116个乳腺肿块术后及穿刺活检病理诊断中良性62个,恶性54个,超声光散射对乳腺癌诊断的敏感度、特异度、准确率、阳性预测值、阴性预测值分别为92.6%、71.0%、81.0%、73.5%、91.7%,超声弹性成像技术分别为77.8%、80.6%、79.3%、77.8%、80.6%,二者联合分别为96.3%、93.5%、94.8%、92.9%、96.7%。结论超声光散射成像系统与超声弹性成像技术各有优势,二者结合有助于提高乳腺病灶鉴别诊断的准确。Objective To compare of diffusion optical tomography and ultrasonic elastography in the differential diagnosis of breast lesions. Methods All patients underwent puncture biopsy or surgical treatment and there is a clear pathological diagnosis. 96 patients with 116 lesions in breast were examined by diffused optical tomography DOT and ultrasonic elastography UE respectively. Results Pathologically there were 62 benign breast lesions and 54 malignant lesions in 116 lesions were confirmed by pathological examination.The sensibility, specificity, accuracy, PPV and NPV of differential diagnosis of benign and malignant breast lumps by DOT were 92.6%, 71.0%, 81.0%, 73.5%, 91.7%, respectively. The corresponding sensibility, specificity and accuracy of by UE were 77.8%, 80.6%, 79.3%, 77.8%, 80.6%, respectively.And the corresponding sensibility specificity and accuracy of by UE combined with DOT were 96.3%, 93.5%, 94.8%, 92.9%, 96.7%, respectively. Conclusion The efficiency of UE in the differential diagnosis of benign and malignant breast lumps is much better than DOT and the combination of UE with DOT is a better solution.
