类风湿关节炎患者血清骨形态发生蛋白6与骨质疏松的关系

摘　　要：目的探讨类风湿关节炎(RA)患者骨形态发生蛋白(BMP)-6与骨质疏松(OP)的关系。方法比较分析99例RA患者与40例健康组及RA患者中不同骨密度组间BMP-6以及其他因子水平,以及各因子之间的相关性。结果 RA患者OP发生率明显高于健康组,其腰椎和股骨颈的骨矿密度(BMD)均低于健康组,而其血清BMP-6浓度明显升高(均<0.05)。RA伴OP的患者BMP-6水平明显升高,抗环瓜氨酸(CCP)抗体水平也明显升高(均<0.05);RA患者的腰椎BMD和股骨颈BMD正相关(=0.817,<0.01),而腰椎BMD与血清BMP-6及年龄负相关(=-0.393、-0.449,均<0.05);股骨颈BMD则与年龄及抗CCP抗体水平负相关(=-0.551、-0.458,均<0.05)。RA患者血清BMP-6浓度与DAS28评分及IL-6、TNF-浓度正相关(=0.249、0.237、0.263,均<0.05)。结论 BMP-6水平异常可能是诱导RA患者发生OP的因素之一,其在RA中的作用机制值得进一步研究探索。Objective To investigate the correlation between osteoporosis （OP） and serum bone morphegenetic protein-6 （BMP-6） in patients with rheumatoid arthritis （RA）. Methods Blood samples from 99 patients with RA and 40 healthy controls were measured by ELISA. The levels of serum BMP-6, interleukin（ IL）-6, IL-1β and tumor necrosis factor（TNF）-α in RA patients and 40 healthy controls, and their correlations to bone mineral density （BMD） and disease activity score for 28 joints （DAS28） were analyzed. Results Occurrence of OP among RA patients was higher than that among healthy controls （P 〈 0.05）. Both BMD of spine and collum femoris were found lower in RA patients than that in heathy controls（P 〈 0.05）, but the level of serum BMP-6 was found higher in RA patients （P 〈 0.05）. RA patients with OP were elder than those with normal BMD. On the other hand, both levels of BMP-6 and anti-CCP antibody （ACPA） were found higher in RA patients with OP than those with normal BMD （P ~ 0.05）. There was no difference between the levels of IL-6, IL-1B and TNF-a in RA patients with or without OP. Spine BMD was positively correlated with collum femoris BMD, but inversely correlated with serum BMP-6 and age（r =0.817, - 0.393,- 0.449; P 〈 0.05）. And coUum femoris BMD was inversely correlated with age and level of ACPA in RA patients （r=- 0.551, - 0.458;P 〈 0.05）. The level of serum BMP-6 was also found positively correlated with DAS28 scores and levels of serum IL-6 and TNF-α in RA patients （r= 0.249, 0.237, 0.263; P 〈 0.05 ）. Conclusions The morbidity of OP in RA patients is high, but the level of serum BMP-6 is high at the same time. The correlation between BMP-6 and OP suggests that the abnormal level of serum BMP-6 maybe is one of the inducers of OP in RA.
