摘 要:目的了解天津市实施新盐碘标准后3类重点人群(孕妇、哺乳期妇女、8~10岁儿童)的碘营养水平,为盐碘含量的调整提供科学依据。方法于2012--2013年选择天津市6个区县,每区县选取2所小学,每所学校随机采集36名8~10岁儿童的随机尿样,并采集每名儿童家庭食用盐;每区县选取孕早、中、晚期的孕妇各30名,哺乳期妇女30名,采集其随机尿样;测定盐碘和尿碘。结果8-10岁儿童、孕妇和哺乳期妇女尿碘中位数分别为180.0、127.2和85.8μg/L,尿碘水平由低到高依次为哺乳期妇女〈孕妇〈8~10岁儿童,其中不同地区儿童碘营养状况均处于适宜水平,孕妇均处于不足状态,沿海区县哺乳期妇女碘营养水平适宜,其他地区均不足。盐碘中位数为23.30mg/kg。结论本次调查的天津市3类重点人群尿碘存在明显的人群和地区差异,孕妇和哺乳期妇女的碘营养有不足的趋势;盐碘中位数低于该市标准(30mg/kg)。Objective To investigate the iodine nutritional level of three special groups (pregnant women,lactating women and children aged 8-10 years) in Tianjin after the implementation of new salt iodine standard and to provide a scientific basis for salt iodine concentration adjustment. Methods In 2012 and 2013, two primary schools were selected in each of the six counties in the city, 25 ml urine of 36 children aged 8-10 years in each school were randomly collected, and 100 g edible salt were collected in every child family.A total of 30 early, middle, late pregnant women and 30 lactating women in each county were selected respectively, the random sample of 25 ml urine were collected. The salt iodine was determined by direct titration, Sichuan salt and other fortified salt by arbitration determination. The urinary iodine was determined by arsenic cerium catalytic spectrophotometry. Results As for 8-10 years old children, pregnant women, lactation women, the medians of urine iodine were 180.0, 127.2 and 85.8 μg/L respectively. The level of urinary iodine of children of different parts was appropriate, and that for the pregnant women was lower than appropriate. The iodine nutrition level of lactation women in coastal counties was appropriate, other parts insufficient. The median of iodized salt iodine was 23.30 mg/kg. Conclusion Urine iodine of three special groups in Tianjin exists obvious differences in people and regions. Iodine nutrition of pregnant and lactation women is insufficient. The median salt iodine concentration is below the standard (30 mg/kg) required in Tianjin. The amount of salt iodine added should be increased to comply with the standard.
