摘 要:目的探讨补肾温阳化瘀方干预子宫内膜异位症痛经的作用机制。方法将75只BALB/c雌性小鼠随机分为假手术组、模型组、中药高剂量组、中药低剂量组与西药组,每组15只。除假手术组外,其余各组小鼠均采用手术移植法复制子宫内膜异位症痛经模型。造模同时中药高、低剂量组分别每日予补肾温阳化瘀方61.67、15.42g/kg和戊酸雌二醇0.5 mg/kg,西药组每周2次予孕三烯酮0.4 mg/kg和戊酸雌二醇0.5mg/kg。于造模12天腹腔注射缩宫素后观察各组小鼠扭体反应,测量异位灶体积,检测小鼠血清血管舒缓激肽(BK)、前列腺素F2α(PGF2α)含量,免疫组化法和western blotting法检测子宫和异位灶缓激肽B1受体(BKB1R)蛋白表达,RT-PCR法检测子宫和异位灶BKB1R mRNA的表达。结果造模第12天,除假手术组外,其余各组小鼠均成功诱发扭体反应并见异位灶。与模型组比较,中药高剂量组、中药低剂量组与西药组扭体潜伏期均明显延长、扭体次数显著减少、异位灶体积减小(P<0.05或P<0.01)。与模型组比较,中药高剂量组、中药低剂量组、西药组血清BK、PGF2α含量显著下降,子宫及异位灶BKB1R蛋白及其mRNA表达显著降低(P<0.01),且中药高剂量组子宫内膜异位灶BKB1R mRNA明显低于西药组(P<0.01)。结论补肾温阳化瘀方能有效防治子宫内膜异位症,其机制可能与降低血清BK及PGF2α含量、抑制BKB1R蛋白及其mRNA的表达有关。Objective To study the mechanism of Kidney-Supplementing Yang-Warming Stasis-Removing Formula (KYSF) for dysmenorrhea caused by endometriosis. Methods Totally 75 male BALB/c mice were randomized into the sham operation group, model group, KYSF high-dose group, KYSF low-dose group and western medicine group, with 15 in each. The mouse models of dysmenorrhea caused by endometriosis were established with surgical transplantation in all groups except the sham operation group. During modeling, the KYSF high-dose group and KYSF low-dose group were given 61.67 g/kg KYSF + 0. 5 mg/kg estradiol valerate or 15.42 g/kg KYSF + 0.5 mg/kg estradiol valerate once daily respectively, and the western medicine group was given 0. 4 mg/kg gestrinone + 0. 5 mg/kg estradiol valerate twice weekly. On the 12th day of modeling, the writhing response and ectopic loci volume was observed, and the contents of bradykinin (BK) and prostaglandin F2α (PGF2α), the expressions of BKB1R protein and BKB1R mRNA in the uterus, and ectopic foci were detected after intraperitoneal injection of oxytocin. Results On the 12th day of modeling, the writhing response and ectopic loci could be seen in all groups except the sham operation group. Comparing with the model group, the latency of writhing was significantly prolonged, writhing times were significantly reduced, and the volume of ectopie foci was significantly decreased in the KYSF groups and western medicine group (P 〈0. 05 or P 〈0.01 ). Comparing with the model group, the contents of BK and PGF2α were significantly decreased, and the expressions of BKB1R protein and BKB1R mRNA in the uterus and ectopic loci were significantly decreased in the KYSF groups and western medicine group (P 〈 0.01 ). Also, the expression of BKB1R mRNA in the KYSF high-dose group was significantly lower than that in the western medicine group ( P 〈 0. 01 ).
