氟比洛芬酯复合利多卡因联合止血带对罗库溴铵注射痛的影响

摘　　要：目的观察氟比洛芬酯复合利多卡因联合止血带对罗库溴铵注射痛的影响。方法择期行气管内插管全身麻醉的患者140例,随机分为生理盐水组(N组)、利多卡因+生理盐水组(L组)、氟比洛芬酯+生理盐水组(F组)和氟比洛芬酯+利多卡因组(FL组),每组35例。所有患者在给药前均先以止血带阻断其局部静脉回流,随后N组给予生理盐水7ml;L组给予2%利多卡因2ml+生理盐水5ml;F组给予氟比洛芬酯5ml(50mg)+生理盐水2ml;FL组给予氟比洛芬酯5ml(50mg)+2%利多卡因2ml。2min后松开止血带,静注罗库溴铵0.6mg/kg。观察和记录患者疼痛程度和术后24h消化道出血、注射部位水肿炎症,头痛头晕、恶心呕吐不良反应。结果与N组比较,L组、F组和FL组患者无痛例数明显增加(P<0.05);与FL组比较,L组和F组患者无痛例数明显减少(P<0.05)。L组、F组和FL组的罗库溴铵注射痛总发生率分别为40.0%、37.1%和11.4%,明显低于N组74.3%(P<0.05);L组和F组明显高于FL组(P<0.05)。术后24h无一例患者出现消化道出血、注射部位水肿、炎症,头痛头晕、恶心呕吐不良反应。结论在罗库溴铵静脉注射前缓慢预注氟比洛芬酯复合利多卡因并联合止血带可有效预防罗库溴铵注射痛的发生,并降低严重程度。Objective To investigate the alleviation effect of vein pretreatment with flurbiprofen axetil combinated with lidocaine and tourniquet on rocuronium injection-induced pain. Methods One hundred and forty patients undergoing general anesthesia with tracheal intubation were randomly allocated into four groups （35 cases in each group）. After the occlusion of venous drainage with tourniquet, patients were pretreated with 7ml of 0. 9% saline in the group N, 5 ml （50 mg） of flurbiprofen axetil and 2 ml of 0.9% saline in the group F, 2 ml （40 mg） of 2% lidocaine and 5 ml of 0. 9% saline in the group L, and 5 ml （50 mg） of flurbiprofen axetil and 2 ml （40 rag） of 2% lidocaine in the group FL, respectively. The occlusion was released 2 min later and then 0. 6 mg/kg rocuronium was injected into the vein. During the injection of rocuronium, the patients were asked to assess and record their pain and adverse reaction during 24 h after operation. Results In comparison with the group N （74.3 %）, the incidence rates of rocuronium injection pain were obviously lower in the group L （40. 0%）, the group F （37.1%） and the group FL （11.4%） （P〈0. 05）, there was a significant reduction in the incidence rates of pain in the group FL compared with the other three groups. None obvious adverse reaction appeared during 24 h after operation. Conclusion Pretreatment with flurbiprofen axetil combined lidocaine and tourniquet before the injection of rocuronium can effectively reduce the incidence and severity of rocuronium injection-induced pain.
