摘 要:目的分析不同情况下不规则抗体鉴定的概率水平。方法采用四格表资料的Fisher确切概率法,结合表格,显示不同情况不规则抗体鉴定概率水平。结果以10个/套谱细胞为例:1)当谱细胞中仅有1个抗原阳性/阴性细胞,即使血清与谱细胞反应和格局表完全一致,也达不到95%的概率水平,报告抗体特异性需慎重;而其他抗原阳性细胞数情况下,血清与谱细胞反应和格局表一致时,均可达到95%的概率水平;2)如血清量不足,只能挑选“半套”谱细胞鉴定抗体,反应细胞总数至少应达到6个(3阳/3阴)或7个(2阳/5阴或2阴/5阳或3阳/4阴或3阴/4阳),方可达到95%的概率水平;3)如抗原抗体反应存在剂量效应,当抗原阳性细胞总数为4~7个时,可允许1个细胞呈假阴性反应。结论掌握不规则抗体鉴定概率水平计算方法,了解不同情况下不规则抗体鉴定的概率,才能在报告抗体特异性时减少因巧合因素所致的错误,保障安全输血。Objective To analyze the probability of irregular antibody identification. Methods Fisher's exact test of four-told table data is used. The probability level of irregular antibody identification is iisted in tables. Results Take 10- cell panel for example:1 ) 95 % probability level cant be reached if only 1 cell's antigen is positive or negative, even if the spectrum of serum is fully consistent with the pattern table of the panel. The antibody specificity should be further identified. 95% probability level can be reached if the antigen of 2 - 8 cells is positive; 2) 6 cells (3 positive/3 negative) or 7 cells (2 positive/5 negative or 5 positive/2 negative ) are at least needed if some cells must be selected for insufficient serum;3 ) when there are 4 - 7 cells with an antigen, 1 cell with false negative response is accepted if dosage effect exists in antigen and antibody. Conclusion To know all kinds of probability level of the irregular antibody identification and its calculation methods could ensure safe transfusion and reduce errors when the antibody specificity is reported.
