盐酸二甲双胍两种剂型治疗2型糖尿病的成本-效果分析

摘　　要：目的:比较盐酸二甲双胍两种剂型治疗2型糖尿病的经济学效果。方法:将126例2型糖尿病患者随机分为两组,A组患者(52例)服用盐酸二甲双胍普通片,B组患者(74例)服用盐酸二甲双胍缓释片,疗程均为12周。观察两组患者治疗前后空腹血糖(FBG)、餐后2 h血糖(2 h PBG)、糖化血红蛋白值(HbA1c),并运用药物经济学评价方法比较其成本-效果。结果:治疗12周后,A组患者有效率为70.21%,FBG下降17.44%,2 h PBG下降23.40%,HbA1c下降14.10%,平均治疗总费用为919.00元,成本-效果比分别为13.09、52.69、39.27和65.18。B组患者有效率为56.52%,FBG下降16.61%,2 h PBG下降18.40%,HbA1c下降10.53%,平均治疗总费用为511.80元,成本-效果比分别为9.06、30.81、27.82和48.60。以B组为参照,A组的增量成本-效果比分别为29.74、490.60、81.44和114.06。结论:两种剂型的盐酸二甲双胍片治疗2型糖尿病疗效均较好,可以改善患者的FBG、2 h PBG、HbA1c,而盐酸二甲双胍缓释片的经济性和依从性较好。OBJECTIVE： To compare the economical effect of 2 dosage forms of metformin hydrochloride in the treatment of type 2 diabetes. METHODS： 126 cases of type 2 diabetes were randomly divided into 2 groups. 52 patients in group A were given Metformin hydrochloride tablet, and 74 patients in group B were given Metformin hydrochloride sustained-release tablet for 12 weeks. The levels of FBG, 2 h PBG and HbA1c were observed in 2 groups belbre and alter treatment; the cost-effectiveness of them were evaluated by pharmacoeconomic method. RESULTS： After 12 weeks, the effective rate of group A was 70.21%; the levels of FBG, 2h PBG and HbA,c decreased by 17.44%, 23.40% and 14.10%, respectively; the total average cost of treatment was 919.00 yuan, and the cost-effectiveness ratios were 13.09, 52.69, 39.27 and 65.18, respectively. The effective rate of group B was 56.52%; the levels of FBG, 2 h PBG and HbA,c decreased by 16.61%, 18.40% and 10.53%; the total average cost of treatment was 511.80 yuan, and the cost-effectiveness ratios were 9.06, 30.81, 27.82 and ,18.60, respectively. And the ratio of incremental cost-effectiveness for group A was 29.74, 490.60, 81.4,1 and 114.06, compared with group B. CONCLUSIONS： Two dosage forms of metformin hydrochloride are effective in the treatment of type 2 diabetes and can improve FBG, 2 h PBG and HbA1c. The cost of Metformin sustained-release tablets is lower and it has better compliance for patients.
