脱细胞真皮基质治疗早期深度烧伤的研究

摘　　要：目的运用异体脱细胞真皮基质(acellular dermal matrix,ADM)修复早期深II^III度烧伤创面,以推广更为有效的治疗方法。方法将36例患者随机分为实验组和对照组,每组18例,对照组采用自身刃厚皮片游离移植结合保痂治疗,实验组采用异体脱细胞真皮基质进行治疗,最后采用温哥华瘢痕量表对患者术后瘢痕的外型、功能及瘢痕再生情况分为3个月、半年及1年进行评价。结果术后半年及1年实验组患者温哥华瘢痕评分分别为(6.02±0.35)分及(5.06±1.37)分,低于对照组的(9.35±1.12)分及(7.38±0.12)分,差异具有统计学意义(P<0.05)。结论运用异体脱细胞真皮基质(ADM)修复深II^III度烧伤创面,患者术后创面恢复良好,瘢痕再生不明显,外观平滑,是值得推广的有效治疗方法。Objective Acellular dermal matrix （ADM） was used to repair the early burn wound depth II-III, to promote a more effective treatment. Methods 36 patients were randomly divided into the experimental group and the control group, and 18 cases in the control group was given its own blade thickness skin graft combined with Paul scab treatment, and the experimental group was given acellular dermal matrix for treatment. Finally Vancouver scale postoperative scar appearance, function and scar regeneration was evaluated after three months, six months and one year. Results The experimental group wound healing time, recovery and appearance function was better than the control group. Conclusion The use of acellular dermal matrix （ADM） to repair deep burn wounds II-III patients recovered well after surgery wounds, and scar regeneration is not obvious, and the appearance of smooth, effective treatment is worth promoting.
