摘 要:目的探讨采用随机扩增DNA多态性(RAPD)技术对临床新生儿血培养标本中分离的光滑假丝酵母菌进行基因分型;分析新生儿感染菌株的同源性,追踪其感染源与传播途径,从而为新生儿假丝酵母菌感染防治提供依据。方法15例新生儿血培养标本中分离的菌株均经过法国科马嘉假丝酵母菌显色鉴定培养基和法国生物梅里埃APl20CAUX酵母菌鉴定试剂盒进行鉴定、纯化,其后采用RAPD多态性分析,获得菌株的RAPD指纹图,应用NTSYS软件计算出相似系(sAB),并进行聚类分析,比较菌株间的同源性。结果15株菌株中14株被鉴定为光滑假丝酵母菌,1株为白色假丝酵母菌,且鉴定分离的2~14号的光滑假丝酵母菌相似系数均≥0.8,表明菌株之间基因型相似度较高,具有基因同源性,而第一株菌株与其他13株之间的相似系数均〈0.6,不具有基因同源性。结论本研究发现新生儿感染的13株光滑假丝酵母菌具有基因同源性,初步证实了试验分离的光滑假丝酵母菌在住院患儿间进行水平传播,提示了光滑假丝酵母菌易引起新生儿院内交叉感染,预防宜采取综合措施,切断传播途径。OBJECTIVE To perform the genotyping of Candida glabrata strains isolated from blood specimens ob- tained from the neonates by using the random amplified polymorphic DNA (RAPD) technique, analyze the homol- ogy of the strains, and track the infection source and transmission routes so as to provide guidance for prevention of the C. glabrata infection in the neonates. METHODS The strains isolated from the blood culture specimens ob- tained from 15 neonates were identified and purified by using the CHROMagar media and API20 CAUX of Bi- oMerieux, France, then further identification was performed by using RAPD so as to acquire the RAPD finger- print of the strains, the SAB was calculated with the use of NTSYS software, the dendrogram was drawn out, and the homology was compared between the strains. RESULTS Of the 15 strains, 14 strains were identified as the C. glabrata, 1 strain as Candida albicans. The similarity coefficients of the isolated C. glabrata strains from No. 2 to No. 14 were no less than 0.8, indicating the genotypes of the strains were highly homological and that the strains had the gene homology. The similarity coefficient between the No. 1 strain and other 13 strains was less than 0.6, indicating that the strains did not have the genetic homology. CONCLUSION All the 13 strains of C. glabrata iden- tified in the neonates have the genetic homology, which primarily confirms that the C. glabrata strains isolated from the experiment transmits horizontally among the hospitalized neonates and indicates that C. glabrata strains are prone to cause hospital-acquired cross infections in the neonates. It is necessary to take comprehensive preven- tion measures to cut off the transmission route.
