提高医务人员手卫生依从率干预效果分析

摘　　要：目的评价综合措施对提升医务人员手卫生依从率效果,探索我国综合医院提升手卫生依从率的有效措施。方法在某三级甲等综合医院76个住院病区和15个医技部门开展多种形式的宣传培训、每月现场调查手卫生依从率并进行多种形式的反馈、将手卫生依从率纳入绩效评估等综合干预措施,比较干预前后手卫生依从率的变化。结果手卫生依从率从干预前的39.63%提升至干预后的82.08%,干预前后差异有统计学意义(χ2=1884.33,P<0.01),各科室、专业人员、手卫生指征的手卫生依从率均提升明显(P<0.01),各类科室中五官科手卫生依从率提升效果最明显,从16.88%提升至86.09%,各类人员中医技人员的提升最明显,从8.00%至76.92%,各手卫生指征中接触患者前提升最明显,从37.98%至81.24%。结论本研究实施的综合干预效果对提升手卫生依从率效果明显,有推广价值,干预前手卫生依从率较低的部门和人群干预后的提升效果更明显。OBJECTIVE To verify the effect of comprehensive interventions on hand hygiene compliance of the medi- cal staff and explore effective measures to improve the hand hygiene compliance in Chinese general hospitals. METHODS The comprehensive interventions, including propaganda and training, onsite survey of hand hygiene compliance and feedback, taking hand hygiene compliance as a performance appraisal indicator, were conducted in 76 inpatient wards and 15 medical techniques departments of a general tertiary hospital, and the change of the hand hygiene compliance was observed and compare before and after the intervention. RESULTS The hand hygiene compliance increased from 39.63~ before the intervention to 82.08% after the intervention, the difference was significant （Xz=1884. 33, P〈0. 001）. The hand hygiene compliance of each department, each professional, and each indicator was increased significantly （P〈0.05）. The hand hygiene compliance increased more significantly in the department of ophthalmology and otorhinolaryngology than in other departments, increasing from 16.88% to 86.09%. Among all the medical staff, the hand hygiene of the medical technicians was increased most significant- ly, increasing from 8. 00% to 76.92%. Among all the hand hygiene indications, the hand hygiene compliance be- fore contacting a patients was increased most significantly, increasing from 37.98% to 81.24%. CONCLUSION The comprehensive interventions can achieve significant effect on improving the hand hygiene compliance and are worthy to be promoted, especially for the departments or personnel with low hand hygiene compliance.
