摘 要:目的比较苯磺酸左旋氨氯地平和缬沙坦对血压和动脉弹性的影响。方法 96例高血压患者被随机分成2组分别服用苯磺酸左旋氨氯地平和缬沙坦,在服药前和服药12个月后测量患者的颈-股脉搏波传导速度(CF-PWV),检测血浆一氧化氮(NO)和内皮素-1(ET-1)的水平。结果降压治疗后2组的收缩压、舒张压、脉压均较治疗前明显降低(P<0.001)。2组CF-PWV在经降压治疗后均有所下降,其中苯磺酸左旋氨氯地平组的幅度明显高于缬沙坦组(2.85±0.67 vs 0.69±0.11,P<0.001)。降压治疗12周后2组患者的NO均较治疗前有所增加,ET-1均降低。其中苯磺酸左旋氨氯地平组NO的增加幅度明显高于缬沙坦组(18.54±8.06 vs 8.64±2.18),ET-1降低的幅度亦高于缬沙坦组(15.64±4.06 vs 3.51±1.14)差异具有统计学意义(P<0.001)。结论本研究结果提示苯磺酸左旋氨氯地平和缬沙坦均能改善高血压患者动脉弹性功能,苯磺酸左旋氨氯地平降低血压和改善动脉弹性的作用更为明显。Objective To compare the effect of levamlodipine and valsartan on improving the artery stiffness in patients with hypertension. Methods Patients with primary hypertension were randomly assigned to levamlodipine or valsartan regimen for 12 weeks. Carotid-femoral pulse wave velocity (CF-PWV) was measured by all automatic device before and 12 weeks after treatment. Serum NO and ET-1 level were analyzed at the same time point. Results SBP, DBP and PP were significantly decreased in both groups (P 〈 0. 001 ). There were no significant changes in HR in both groups ( P 〉 0. 05 ). CF-PWV and ET-1 significantly decreased while NO significantly increased in both groups. The change of CF-PWV, NO and ET-1 were greater in levamlodipine group than in valsartan group ( P 〈 0. 001 ). Conclusion Arterial stiffness of hypertensive patients were improved post levamlodipine and valsartan therapy. Levamlodipine is superior to valsartan on reducing blood pressure and improving arterial stiffness of hypertensive patients.
