宫腔镜治疗子宫内膜息肉58例疗效分析

摘　　要：目的 探讨宫腔镜治疗子宫内膜息肉的临床疗效及术后复发率.方法 对2011年1月至2011年12月因子宫异常出血来本院就诊,经B超及宫腔镜检查诊断为子宫内膜息肉的58例患者行宫腔镜子宫内膜息肉电切术.记录患者手术时间、出血量,术后定期随访1年,追踪患者术后月经变化、贫血恢复情况及子宫内膜息肉复发情况.结果 58例患者均顺利完成手术,手术时间为10 ～ 18 min,平均（12.0± 1.3） min,术中出血量5～15ml,平均（8.0±2.5）ml,手术一次成功率为100.0％,无手术并发症发生.术后随访1年,月经量增多者的治疗有效率为95.2％,不规则阴道流血者的治疗有效率为93.7％.因月经量过多致贫血者术后随访1～3月血色素明显上升,基本恢复正常.所有患者术后1年复查B超及宫腔镜检查均未发现子宫内膜息肉复发.结论 宫腔镜子宫内膜息肉电切术是治疗子宫内膜息肉的有效方法,具有手术时间短、出血少、损伤小、术后复发率低、疗效确切等特点,值得临床广泛推广应用.Objective To explore the clinical efficacy of hysteroscopy in the treatment of endometrial polyp and its recurrence rate.Methods Hysteroscopic endometrial polyp electrocision was performed on 58 patients who came to our hospital because of abnormal uterine bleeding from January to December,2011 and were diagnosed with endometrial polyp with type-B ultrasonic and hysteroscopic examination.The operation time and bleeding volume were recorded.The patients were followed up for 1 year to understand the patients＇ menstrual changes,state of the recovery from anaemia,and recurrence rate of endometrial polyp.Results The operation was successful in one time in all patients.The operation time was 10 to 18 minutes with an average of （12.0 ± 1.3） minutes.The bleeding volume was 5 to 15 ml with an average of （8.0 ± 2.5） ml.No complication occurred.The follow-up showed that the cure rates of menorrhagia and irregular vaginal bleeding were 95.2％ and 93.7％,respectively.The hemochromatosis was markedly increased to almost normal level in the patients with anemia caused by menorrhagia.The type-B ultrasonic and hysteroscope examination indicated that no case recurred.Conclusions Hysteroscopic electrocision in the treatment of endometrial polyp is effective,has short operation time,low bleeding volume,and low recurrence rate,and is worth being clinically generalized.
