连花清瘟胶囊治疗慢性阻塞性肺疾病急性加重期的临床疗效和作用机制

摘　　要：目的探讨连花清瘟胶囊(连翘、金银花、炙麻黄等)对慢性阻塞性肺疾病急性加重期患者的临床疗效及对IL-8、TNF-α水平的影响。方法将60例慢阻肺急性加重期患者分为连花清瘟治疗组和常规治疗组,每组各30例。两组患者均予以基础治疗,对连花清瘟治疗组患者加用连花清瘟胶囊,评估临床疗效以及肺功能、血和痰IL-8、TNF-α水平的变化。结果连花清瘟治疗组患者治疗后临床疗效较常规治疗组改善显著(P<0.01)。与治疗前比较,连花清瘟治疗组患者肺功能好转,血清IL-8水平降低(P<0.05),痰IL-8、TNF-α水平显著下降(P<0.01),血TNF-α未出现有统计学意义的变化(P>0.05),而常规治疗组仅痰TNF-α较前下降(P<0.05)。结论连花清瘟胶囊可以显著改善慢性阻塞性肺疾病急性加重期患者的临床疗效,其作用机制可能是通过抑制相关炎症因子IL-8、TNF-α的释放。AIM To explore the clinical effect of Lianhua Qingwen Capsules（ LHQWC,Forsythiae Fructus, Lonicerae japonicae Flos,Ephedrae Herba,etc.） on patients with acute exacerbation of chronic obstructive pulmonary disease（ COPD） and the influence of IL-8 and TNF-α level. METHODS Sixty COPD patients with an acute exacerbation were divided into LHQWC therapy group（ conventional therapy plus LHQWC） and conventional therapy group. The clinical effect,lung function,changes of IL-8 and TNF-α level in blood serum and sputum were evaluated in all of the patients. RESULTS The clinical effect of the LHQWC therapy group was dramatically improved as compared with that of the conventional therapy group（ P〈0. 01）. In contrast with pretreatment,the patients in LHQWC therapy group had better lung function and lower IL-8 level in blood serum（ P〈0. 05）,but no significant change of blood TNF-α. The expression of sputum IL-8 and TNF-α declined sharply after treatment with LHQW（ P〈0. 01）,while only sputum TNF-α level in conventional therapy group dropped（ P〈0. 05）. CONCLUSION LHQWC has a obviously clinical effect during acute exacerbation of COPD. Its mechanism may be related to the release of IL-8 and TNF-α.
