摘  要:目的用彩色多普勒超声(CDUS)监测糖尿病肾病患者NADPH氧化酶p22phox亚基多态性在左肾动脉(LRA)血流动力学方面的改变。方法应用聚合酶链反应-限制性片段长度多态性技术对186例糖尿病肾病患者的NADPH氧化酶p22phox亚基242位点进行基因型分析,将他们分为CC基因型和CT+TT基因型2组,应用CDUS测量其LRA最大血流速度(Vmax)、最小血流速度(Vmin)、平均血流速度(Vmean)、搏动指数(PI)、阻力指数(RI)等血流动力学有关参数。结果 CC基因型糖尿病肾病患者LRA Vmax、Vmin、Vmean、Pl、RI增加,与CT+TT基因型糖尿病肾病患者比较差异有统计学意义(P<0.05,P<0.01)。结论 NADPH氧化酶p22phox亚基242位点基因多态性与糖尿病肾病患者肾脏血流动力学改变存在相关性。CDUS通过监测糖尿病肾病患者肾脏血流动力学改变,为其早期诊断和预后评估提供了一种无创监测的方法。Objective The purpose of this research was to evaluate the application of Doppler ultrasound for monitoring NADPH oxidase subunit p22phox gene C242T polymorphism in hemodynamic changes of left renal artery (LRA) among diabetic nephropathy (DN) patients. Methods Genotypes of NADPH oxidase subunit p22phox were typed in 186 DN patients by polymerase chain reaction and restriction fragment length polymorphism. Then the patients were divided in two groups, which were CC genotype and CT + TT genotype. The maximal systolic velocity (Vmax), the minimal diastolic velocity (Vmin), the mean velocity (Vmean), the resistance index (RI) and the pulsatility index (PI) of the LRA were measured with Doppler ultrasound. Results Vmax, Vmin, Vmean, RI and PI were increased in CC genotype, and there was significant difference compared to the CT + TT genotype (P〈0.05, P〈0.01). Conclusion The distribution of NADPH oxidase subunit p22phox gene C242T polymorphism was related to hemodynamic changes of left renal artery (LRA) in DN patients. Doppler ultrasound can be used to non-invasively monitor the hemodynamic changes of LRA in DN patients.
