摘  要:目的探讨磁共振T1ρ和T2mapping成像评价骨性关节炎(OA)早期软骨损伤的价值。方法对78例OA患者(轻度OA组50例,重度OA组28例)和23名正常人(正常组)行膝关节软骨MR扫描,包括FSE T1W/T2W、自旋锁定T1ρ和T2mapping成像。分别测量股骨软骨平均T1ρ和T2值,采用SPSS 18.0统计学软件比较各组间的差异。结果共86个膝关节516个观察面纳入研究,其中正常组20名,轻度OA组41例,重度OA组25例。65例(143个关节面)出现不同程度软骨损伤,主要位于股骨内侧髁后部。重度OA、轻度OA及正常组平均T1ρ和T2值分别为(48.37±5.80)ms和(48.67±6.05)ms、(44.26±4.16)ms和(45.78±5.85)ms、(40.34±2.10)ms和(42.62±4.15)ms,各组间T1ρ值和T2值差异均有统计学差异(P<0.05),重度OA组与正常组T2值差异有统计学差异(P<0.05)。结论 T1ρ和T2mapping成像可发现早期软骨损伤,T1ρ敏感度更高,可用于早期诊断骨性关节炎。Objective To observe the value of MR T1ρ and T2 mapping for detecting cartilage degeneration in early osteo-arthritis patients (OA). Methods Totally 78 OA patients (mild OA group: n=50; severe OA group: n=28) and 23 nor real subjects (normal group) underwent 3.0T MR scanning, including FSE T1W, T2W and spin lock T1ρ and T2 mapping imaging. T1ρ and T2 values were measured and compared with SPSS 18. 0 statistical software. Results A total of 86 knees and 516 articular surfaces were enrolled. Cartilage damages in different degrees were detected in 65 patients (143 articular surfaces), most of the damages were found at the back of the medial femoral condyle. The average T1ρ and T2 values of se-vere, mild and normal group was (48.37±5.80)ms and (48.67±6.05)ms, (44.26±4.16)ms and (45.78±5.85)ms, (40. 34±2.10)ms and (42.62±4.15)ms, respectively (P〈0. 05). T1ρ was statistically different among 3 groups (P〈 0.05), T2 values were statistically different between severe OA group and normal group (P〈 0.05). Conclusion T1ρ and T2 mapping can be used as non-invasive evaluation method for on-set cartilage disease. T10 may be a more sensitive indica-tor for early OA.
