超声引导在肾组织活检中的准确性和安全性分析

摘　　要：目的 探讨超声引导下肾组织活检的准确性和安全性.方法 对该院2011年1月-2013年1月进行肾组织穿刺活检的242例病例进行回顾性分析,其中超声组197例为超声引导下穿刺,CT组45例为CT引导下穿刺,比较其穿刺的准确性和术后并发症情况.结果 超声组和CT组的成功率分别为93.91％、97.78％.前两次穿刺组间比较时发现,CT组的每次穿刺成功率高于超声组,且差异有统计学意义（P＜0.05）;而总体成功率比较时,两组差异不明显（P＞0.05）.对并发症的总体情况进行组间比较时发现,两组并发症发生率无显著差异（P＞0.05）.其中肉眼血尿患者于1 ～3d后血尿消失;肾周血肿2例,均未作特殊处理,自行好转.结论 超声引导下肾组织活检能清晰地显示肾脏组织的断面结构,实时动态地监测活检针穿刺及取材的全过程,定位准确、操作简单、安全性高,在临床上具有较高的应用价值.Abstract：Objective To explore the accuracy and security of ultrasound-guided renal biopsy. Methods 242 cases of kidney biopsy admitted to our hospital from January 2011 to January 2013 were retrospectively analyzed, including ultrasound group 197 cases of ultrasound-guided biopsy, CT group,45 cases of CT-guided biopsy. The puncture accuracy and postoperative complications were compared. Results Ultrasound group and the CT group＇s success rates were 93.91% ,97.78%. The inter-group comparison in the first two pantures showed that, each puncture success rate in CT group was higher than that in ultrasound group, and the difference was statistically significant （P 〈 0. 05 ） ; while the overall success rate was not significantly different between the two groups （ P 〉 0.05 ）. The overall incidences of complications between the two groups were not significantly different（P 〉 0.05 ）. Among patients with gross hematuria hematuria disappeared in 1 - 3 d. Perirenal hematoma was found in two cases, which improved without special treatment. Conclusions Ultrasound-guided renal biopsy can clearly show kidney tissue section structure, real-time dynamically monitoring the whole process of biopsy,which has the advantapes of accurate positioning, simple operation, high safety, and high clinical application value.
